(Marston Rd) to existing 4-lanes Pre-Construction Conference Minutes In accordance with Article 108-3 of the Standard Specifications, a Preconstruction Conference was held for the above referenced project on Tuesday, November 25, 2008 at 10:00 am at the Division Engineer's Office in Aberdeen, North Carolina. Utility Items: • R/W has been acquired for all utilities throughout the projects. • Post office to be moved out by January 1, 2009. • Bill boards may be taken down. • Some concrete foundations /pads are with in the RIW. Contractor noted this will not be a problem to remove. Contractor noted the Mr. J. J. Harren will be the Utility Coordinator for R. E. Goodson. • Ed Reams noted that at Station +1 -179 there is a fiber line that cannot be cut. • Progress Energy has five clearing crews working at this time. • Time Warner did not have anyone present. All facilities to be relocated by August 1, 2009. • Windstream is waiting for Progress Energy to finish their work. All facilities to be relocated by February 1, 2010. • AT &T is also waiting on Progress Energy to complete their work. All facilities to be relocated by October 1, 2009. • No one was present for Verizon. There is a conflict with 30" drain pipe at station 179 rt. --L -. Per contract Verizon requests a 3 week notice and 1 week construction time to bore and jack installation. • A meeting to be set up with all utility companies, the Department and the project utility coordinator for Thursday December 4, 2008 at 10:00 am. Permits/ Erosion Control: • Contractor to have certified erosion and sediment control stormwater supervisor, certified foremen, certified installers and certified designer. • John Olinger stress that erosion control is a very serious issue and to keep plans on -site and up to date. • Department would like to make sure the survey party has a clear understanding of the flagging at bridge sites. There is some hand clearing at this site. • Mr. Spencer with the USACE stated the importance of not deviating from what the permits show on the plans. • Use Best Management Practices to minimize silt loss. • If any erosion control issues arise bring them up and address them immediately. • If we find out the permits differ from contract make Marty Tillman or Ron Van Cleef aware so we can get it resolved in a timely manner. Mr. Goodson Preconstruction Minutes January 30, 2009 Page 3 Permits/ Erosion Control: (Cont) • Keep a rain gauge and log book on site. • Orange fence to be installed at wet lands area. • Keep weekly reports up to date. • When measures are installed highlight in yellow and date. • Make notes of any concern or blowouts so they can be addressed when roadside visits project. General Items: • Contractor submitted a tentative progress schedule. This schedule will be updated after the meeting with utility companies. Mr. Olinger advised Contractor no to exceed payout schedule in contract. Mr. James Goodson stated they were working on the subcontract agreements and are forthcoming. • R. E. Goodson plans to start work sometime before Christmas. • Project manager will be Mr. Torn Goodson. • Traffic control coordinator will be Mr. Brett Calcutt. • Liquidated Damages are $2,800.00 per calendar day. • ICT 1 includes all major holidays and states that the contractor shall not narrow or close a lane of traffic on US 1 and all —Y- lines, detain and / or alter the traffic flow on or during holiday weekends, special events, or any other time when traffic is unusually heavy. • ICT 2 and 3 gives 30 days for each phase II steps 3A, 2A and has liquidated damages of $500.00 per calendar day. • ICT 4 gives 72 hours for phase III step 2 and has liquidated damages of $500.00 per hour. • Contractor to report all MBE's and WBE's. If a problem occurs with any minority businesses please report it to Marty Tillman or Ron Van Cleef. Follow all guidelines when replacing a minority business. • John Olinger supplied the contractor a claim submittal form. • This project has a twelve month guarantee. • Prior to any work being done contractor to contact CSX representative Mr. J. D. Kirkland at (919) 362 -9402. A 30" welded steel pipe under the tracks of CSX railway has a wall thickness of 0.079 ". Department to look into this issue. • Riley paving to cut cores for density. • Mr. Olinger advised contractor to use a video camera to video tape project incase any problems may arise in the future. Contractor to install temporary chain link fence prior to removing existing fence at parcel 26. • This contract does have a seeding incentive. • Mr. Ron Shaw said they plan to use pipe piles for the temporary bridges and plan to use a barge and mats to build the new structures. • John Olinger stated we would be using CSL testing on some of the shafts. • Mr. Shaw plans to send submittals straight to Raleigh. Copy of cover letter needs to be send to Resident Engineer's Office. • Mr. Tillman stated that class 11 safety vest shall be worn by all contractors personnel and subcontractors. • Back up alarms on trucks is a must. If a truck shows up with out a back up alarm they will be asked to leave the project. • When trenching and shoring a competent person must be onsite. Mr. Goodson Preconstruction Minutes January 30, 2009 Page 4 • Keep all intersections clear of site for traveling public as well as guiding through the project in a safe manner. • Waterline specs will be the Departments specifications but we will also work with the owners. • When submitting supplemental agreements please provide the Department with a cost breakdown of materials and labor. • The Department to provide R. E. Goodson with electronic files to make model for grading purposes. These minutes are prepared as they are documented. Any discrepancies with the content of these minutes should be submitted in writing to this office. Failure to do so will be an indication of agreement. Sincerely, /�Jon R.
