3. Project Description:
3a.Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this
application:
A 48"RCP culvert approximately 400 ft NE of the Cole Mill Rd bridge(#310049)in
the DOT ROW of Cole Mill Rd became blocked by debris and overtopped at some
unknown point in time.The overtopped culvert caused large erosional gullies to
form that begin at the bridge abutment and continue downslope to the left bank of
the Eno River.
The Cole Mill Rd(SR-1401)bridge became a popular,unauthorized access point to
Eno River State Park's Pea Creek Trail when the Park closed due to Covid-19.
Wooden steps of unknown origin exist down the bridge slope next to the large
gullies that visitors use to access the closed Pea Creek Trail,and park on the side
of Cole Mill Rd when doing so. DOT has agreed to assist State Parks in repairing
the slope and removing the steps to discourage this dangerous roadside parking
and unauthorized access to an unsafe trail that has unfortunately exacerbated due
to the pandemic.

4. Proposed Activity
Provide a detailed description of the proposed activity including its purpose and include the type of equipment to be used:*
DOT will stabilize an approximate 35 ft wide section of the bridge slope between the
large erosional gullies and wooden steps with rip rap and check dams to improve
erosion&sedimentation control measures.The wooden steps and 3 tulip poplars
will be removed when check dams are installed.DOT is providing materials and
labor while State Parks will obtain the buffer permit.The left top of bank has not yet
eroded and therefore the closest rip rap placement will be—15 ft landward and will
not impact the OHWM.

NCDOT BMPs will be utilized to avoid adverse impacts to the Eno River. Manual
labor will be performed to the maximum extent practical to avoid excesssive
equipment tracking down the slope.
lb.Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques.*
The wooden steps will not be removed until just before stabilization work begins.
This will avoid concentrated flow paths from forming between the gullies and bare
earth slope.The culvert upstream that caused the accelerated runoff issues has
already been cleared out by DOT and will better handle stormwater for the drainage
area.
