Fayetteville Regional Office on 10 January 2021, we are forwarding herewith Well Construction Permit No.
WM0601167 dated 15 January 2021 issued to the Division of Waste Management/UST Section for the
construction of one (1) monitoring well located on property owned by Glenn Brewer (P.I.N. #: 852900218458)
in Eagle Springs, North Carolina in association with the Brewer's Repair Shop investigation located at 8325
NC Hwy 705, Eagle Springs, North Carolina.
This Permit will be effective from the date of its issuance for one year and shall be subject to the
conditions and limitations as specified therein.
Please note that according to North Carolina Administrative Code, Title 15A, Subchapter 2C, Section
.0105 (g), "it is the responsibility of the well owner or his agent to see that a permit is secured prior to the
commencement of construction of any well for which a permit is required."
Issuance of this permit does not constitute approval of the subject wells for reimbursement from Trust
Funds.
If any parts, requirements, or limitations contained in this Permit are unacceptable to you, you have the
right to an adjudicatory hearing before a hearing officer upon written demand to the Director within 30
days following receipt of this Permit, identifying the specific issues to be contended. Unless such demand
is made, this Permit shall be final and binding.

In accordance with the provisions of Article 7, Chapter 87, North Carolina General Statutes, and other
applicable Laws, Rules and Regulations.
PERMISSION IS HEREBY GRANTED TO
DIVISION OF WASTE MANAGEMENT/UST SECTION
FOR THE CONSTRUCTION OF A MONITORING WELL SYSTEM consisting of one (1) monitoring
well owned by the Division of Waste Management/UST Section located at 217 West Jones Street, Raleigh,
North Carolina. The monitoring well will be located on property owned by Glenn Brewer at 8308 NC Hwy
705 (P.I.N. #: 825900218458) in Eagle Springs, Moore County, North Carolina. This Permit is issued in
accordance with the application received on 10 January 2021 in conformity with specifications and
supporting data, all of which are filed with the Department of Environmental Quality and are considered
integral parts of this Permit.
This Permit is for well construction only, and does not waive any provision or requirement of any other
applicable law or regulation. Construction of any well under this Permit shall be in strict compliance with the
North Carolina Well Construction Regulations and Standards (15A NCAC 02C .0100), and other State and
Local Laws and regulations pertaining to well construction.
If any requirements or limitations specified in this Permit are unacceptable, you have a right to an adjudicatory
hearing upon written request within 30 days of receipt of this Permit. The request must be in the form of a
written petition conforming to Chapter 150B of the North Carolina General Statutes and filed with the Office
of Administrative Hearings, 6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Unless such a
demand is made, this Permit is final and binding.
This Permit will be effective for one year from the date of its issuance and shall be subject to other specified
conditions, limitations, or exceptions as follows:
1. Issuance of this Permit does not obligate reimbursement from State trust funds, if these wells are being
installed as part of an investigation for contamination from an underground storage tank or dry cleaner
incident.
2. Issuance of this Permit does not supersede any other agreement, permit, or requirement issued by
another agency.
3. The well(s) shall be located and constructed as shown on the attachments submitted as part of the
Permit application.
4. Each well shall have a Well Contractor Identification Plate in accordance with 15A NCAC 02C
.0108(o).
5. Well construction records (GW-1) for each well shall be submitted to the Division of Water Resources
Information Processing Unit within 30 days of the well completion.
6. When the well is discontinued or abandoned, it shall be abandoned in accordance with 15A NCAC
02C .0113 and a well abandonment record (GW-30) shall be submitted to the Division of Water
Resources Information Processing Unit within 30 days of the well abandonment.

Dear Mr. Brewer, Ms. Brewer, & Mr. Brewer,
WithersRavenel (WR) is under contract with the North Carolina Department of Environmental Quality
(NCDEQ), Underground Storage Tank (UST) Section, Federal Trust Fund (FTF) Branch for the assessment of
leaking underground storage tanks (LUSTs). NCDEQ has tasked WR with completing assessment and
monitoring activities at the Brewer's Repair Shop site, located at 8325 NC Hwy 705 in Eagle Springs, Moore
County, NC. Based on research of property ownership in the vicinity of the site, WR has learned that you
are the owner of a parcel located to the North of the property.
As a part of assessment activities at the site, NCDEQ has tasked WR with determining whether groundwater
contamination is present and monitoring groundwater contamination on the site and nearby properties. For
this reason, WR is requesting your permission one permanent groundwater monitoring well on your property
(Moore County Parcel # 00007745), located at 8308 NC Hwy 705 in Eagle Springs, NC for the purposes of
monitoring groundwater conditions at the location. The proposed groundwater monitoring well is replacing
a former monitoring well that was previously located on your property. The installation of the permanent
well will be completed in a single day. Installation of the permanent well will involve drilling with a small,
track -mounted drill rig and the placement of 2-inch diameter PVC screen and riser at the location. The well
will be secured at ground surface with a locking cap and would include a flush -mounted manhole set in a 2-
feet by 2-feet concrete pad. The proposed location of the permanent groundwater monitoring well is shown
on Figure 1 (attached) but might change slightly based on actual conditions (e.g., buried or overhead utilities).
NCDEQ has requested that we approach you for the purposes of obtaining an access agreement which will
allow us to complete the monitoring and assessment work on your property. Any and all fees for this
monitoring and assessment are the responsibility of NCDEQ, and you will incur no charges of any kind for
this work. I have included an access agreement for your consideration, and I request that you complete and
return the agreement in the enclosed, pre -addressed, and stamped envelope. I have also included a copy of
the agreement for your records.

its contractor, to enter upon said property for the purposes of conducting an investigation of the
groundwaters under the authority of G.S. 143-215.3(a)2.

1. The investigation shall be conducted by the UST Section of the Department's Division of Waste
Management or its contractor.
2. The costs of construction and maintenance of the site and access shall be borne by the
Department or its contractor. The Department or its contractor shall protect and prevent
damage to the surrounding lands.
3. Unless otherwise agreed, the Department or its contractor shall have access to the site by the
shortest feasible route to the nearest public road. The Department or its contractor may enter
upon the land at reasonable times and have full right of access during the period of the
investigation.
4. Any claims which may arise against the Department or its contractor shall be governed by Article
31 of Chapter 143 of the North Carolina General Statutes, Tort Claims Against State
Departments and Agencies, and as otherwise provided by law.
5. The information derived from the investigation shall be made available to the owner upon
request and is a public record, in accordance with G.S. 132-1.
