This Permit will be effective from the date of its issuance for one year and shall be subject to the conditions and limitations as specified therein. Please note that according to North Carolina Administrative Code, Title 15A, Subchapter 2C, Section .0105 (g), "it is the responsibility of the well owner or his agent to see that a permit is secured prior to the commencement of construction of any well for which a permit is required." Issuance of this permit does not constitute approval of the subject wells for reimbursement from Trust Funds. If any parts, requirements, or limitations contained in this Permit are unacceptable to you, you have the right to an adjudicatory hearing before a hearing officer upon written demand to the Director within 30 days following receipt of this Permit, identifying the specific issues to be contended. Construction of any well under this Permit shall be in strict compliance with the North Carolina Well Construction Regulations and Standards (15A NCAC 02C .0100), and other State and Local Laws and regulations pertaining to well construction. When the well is discontinued or abandoned, it shall be abandoned in accordance with 15A NCAC 02C .0113 and a well abandonment record (GW-30) shall be submitted to the Division of Water Resources Information Processing Unit within 30 days of the well abandonment. NCDEQ has tasked WR with completing assessment and monitoring activities at the Brewer's Repair Shop site, located at 8325 NC Hwy 705 in Eagle Springs, Moore County, NC. Based on research of property ownership in the vicinity of the site, WR has learned that you are the owner of a parcel located to the North of the property. As a part of assessment activities at the site, NCDEQ has tasked WR with determining whether groundwater contamination is present and monitoring groundwater contamination on the site and nearby properties. For this reason, WR is requesting your permission one permanent groundwater monitoring well on your property (Moore County Parcel # 00007745), located at 8308 NC Hwy 705 in Eagle Springs, NC for the purposes of monitoring groundwater conditions at the location. The proposed groundwater monitoring well is replacing a former monitoring well that was previously located on your property. The installation of the permanent well will be completed in a single day. Installation of the permanent well will involve drilling with a small, track -mounted drill rig and the placement of 2-inch diameter PVC screen and riser at the location. The well will be secured at ground surface with a locking cap and would include a flush -mounted manhole set in a 2- feet by 2-feet concrete pad. The proposed location of the permanent groundwater monitoring well is shown on Figure 1 (attached) but might change slightly based on actual conditions (e.g., buried or overhead utilities). NCDEQ has requested that we approach you for the purposes of obtaining an access agreement which will allow us to complete the monitoring and assessment work on your property. Any and all fees for this monitoring and assessment are the responsibility of NCDEQ, and you will incur no charges of any kind for this work.
