1. The streets to be constructed for this project are subject to the Department of
Transportation Certification Program and Quality Management System
Maintenance Version Requirements. The Contractor, at his expense, shall provide
test results on 1) Subgrade, 2) Base and 3) Pavement. The Contractor shall insure
that all construction and testing is performed in accordance with the N.C.
Department of Transportation "Standard Specifications for Roads and Structures",
latest edition, 2006 Hot Mix Asphalt Specifications and Manual, and "Quality
Management System Maintenance Version", and shall report the testing and /or
core locations, test method, results and D.O.T. allowable range or tolerance, as
applicable. Subgrade shall be tested for density and shall be proofrolled by the
testing Engineer or the Engineer or Surveyor who will issue the Final Certification.
Base and pavement shall both be tested for density and thickness. If Parker &
Associates is administering the construction they must also proof roll the stone.
Twenty-four (24) hour advance scheduling before proof roll. Testing for pavement
shall be certified by either a professional Engineer or properly certified QMS
technician and shall be in accordance with QMS criteria. €tequired thickness for this
project is as shown on the plans. Note that thickness for widening or turn lanes is
greater than that for internal streets.

3. Notify the Engineer or Surveyor, who will issue the overall Final
Certification, of all phases of Roadway Construction.
4. All metal used for any type of catch basin or inlet shall be produced in the USA.
5. A minimum sight distance of 10'X70' shall be maintained
along the right-of-way of all internal intersections.
6.20 foot radii at right-of-way intersections, 30 foot radii at
edge of pavement intersections.
7. All culverts under streets shall be reinforced concrete per North Carolina Department
of Transportation Specifications, unless otherwise specified and/or approved by the
Engineer and/or Department of Transportation.
8. All roads shall be public and built to NCOOT Standards.
9. There shall be no encumbrances within the NCDOT Right-of-way.
10. All materials and practices shall meet North Carolina Department
of Transportation Standards.

Contractor shall insure that he and all his workers (contractors,
subcontractors and other site personnel) do not damage construction
stakes or other measures used in laying out the project whether by
the Engineer or another entity including those under the employ of the
contractor. Contractor shall cause his workers to observe and verify
to the fullest extent practicable that stakes, other measures and grades
are, or appear to be, accurate and correct, and immediately, but prior
to any associated work, notify the staking entity of any potential conflict,
error or question regarding the work, The Engineer shall not be responsible
if all of the above measures are not strictly adhered to. The Engineer shall
not be responsible where due to damage, his staking can not be verified.
For critical or costly components of the work, where the contractor believes
that staking may not be preserved, the contractor shall request the staking
entity to place staking in an area where its preservation can be insured.

1. Potable water, supply and wastewater treatment to be by ONWASA_
2. Contractor shall verify location of existing water & sewer mains & report any
deviations to Engineer prior to construction_
3. As of October 1, 2014, anyone digging with mechanized equipment in highway
right-of-way, private utility easements, or public spaces will be required by North
Carolina law to give notice of the proposed excavation to all existing area utilities at
least three to twelve business days before starting to dig. The utility owner is to
locate its facilities in the area of the proposed excavation. Utility members of North
Carolina 811 can be contacted at 1-800-632-4949,
4. Contractor is responsible for contacting any utility company that is not a
member of N.C. One Call.
5. Contractor shall verify location of all underground utilities prior to
commencement of work.
6. Contractor is responsible for any damages to any existing utilities.
7. All proposed sewer mains shall be SDR PVC 35, PVC C900 or Ductile Iron as shown
on plans. Services to be 4-inch Sch. 40 PVC or Ductile Iron. Place 3" to 6"
minimum #57 stone bedding under all gravity mains. Place #57 stone hounching
to V above (min) above top of pvc pipe and to spring line (min.) for ductile iron.
Place 12" compacted select fill above stone to 12" minimum above pipe, then
suitable fill compacted in 6" lifts above that.
8. All manholes located outside of street right-of-ways shall have watertight rings
& covers_
9. All water mains 6" through 12" shall be AWWA C-900, Class 150.2" water mains
shall be ASTM D-2241, PVC 1120, SDR 21, 200 PSI pressure rating.
10. Fire hydrants shall be Mueller Centurion Model A421 or American Flow Control
Mark 73.
11. Water mains shall be buried with a minimum of 36 inches cover, unless otherwise
noted.
12. Water mains & sewage mains shall have proper separations. At water & sewer
crossings , the water main shall have 18 inches separation above the sewer main or
they both shall be constructed of ferrous material for a minimum of 9 feet each
side of the crossing. See specifications for further details.
13, Contractor to lay water mains at extra depth to avoid conflicts with storm
drainage and other facilities.
14, Contractor shall notify The Certifying Engineer and ONWASA at least 48 hours
prior to commencing any work, inspections, sampling, & pressure tests.
15. Chlorination & pressure tests are required in the presence of The Certifying
Engineer and ONWASA.
16. Contractor shall furnish negative bacterial analysis results A 24 hour chlorine
residual results to The Certifying Engineer and ONWASA.
17. Contractor shall maintain comprehensive (depth & horizontal locations) field
"As guilts" for all installations & submit them to the Certifying Engineer & ONWASA_
18. All units shall have individual water & sewer services installed by the contractor.

specifications.
20. All water A sewer system installation materials & practices shall meet ONWASA
specifications_
21. No construction of either water or sewer mains is to begin prior to receipt of
permits to construct from NCDENR in accordance with 15A NCAC18C.03'05 (a) and
15A NCACO2H.02'04.15A NCAC18C.03'05 (a) and 15A NCACO2H.02'04.
22. All lots are less than 5 miles from the Piney Green Fire Station.

1. Contractor to field verify location and depths of existing water mains
and any other utilities prior to beginning bore.
2. Casing thickness shall be per DOT requirements. Steel encasement pipe
shall conform to ASTM A-139 with wall thickness and grade as defined
in the specifications. minimum allowable yield strength is 35,000 psi.
3. Bore and Receiver Pit shall be a minimum of f H per V V depth away
from the cop.
4. Carrier Pipe shall be adequately supported the entire length within the
casing by using "spider" steel supports at a maximum of 9 foot centers
(one at each joint and one intermediate). Other methods must meet
approval of the Engineer.
5. Materials and installation shall meet ONWA5A and NCDOT specifications.

Erosion and Sedimentation Control Notes:
1. Construction Sequence:
A. Notify Engineer, Owner, and NCDEQ prior to beginning construction.
R. Clear only the area necessary to install the construction entrances and perimeter silt fences, sediment
traps and sediment basins. The remainder and bulk of clearing is not to be done until installation of these
items has been completed-
C. Sediment control measures are required to be installed at the earliest practicable date after start of
work Grading operations shall begin at tow points, points of outfall release or at other locations of where
sediment control measures are shown on plan, and the sediment control devices (silt fences, sediment
traps, sediment basins, etc.) shall be installed before or simultaneously with the grading of any area
draining to the device.
D. Install stabilized construction entrance prior to beginning construction.
E_ Clear and grub entire project area.
F. Install silt fence, sediment traps and sediment basins.
G. Rough grade and temporary seed areas not to be disturbed during street and utility construction.
H. Install Swales
I. Install storm drain pipes and outlet protection.
J_ Rough grade roads and roadside swales.
K Install water mains.
L Fine grade streets. Place CABC over compacted subgrade. Contractor must conduct and pass a proof roll
test, on the subgrade prior to installing stone, and on the stone prior to installing asphalt. The proof roll
tests shall be conducted in the presence of The Soil Engineer. Density and thickness tests on subgr ode,
stone and asphalt by The Soil Engineer shall be conducted and approved and tests results submitted to
Engineer. For additional info, See Roads and Drainage notes.
M. Maintain erosion control measures as needed to assure full functionability.
N. Install asphalt surface course. Soil Engineer to witness laydown and test for thickness and density and
submit results to Engineer.
O. Temporary or permanent seed, fertilize, and mulch ail disturbed areas after completing any phase of the
grading work within the fallowing time schedule. Apply temporary seeding and mulch if area is to be
disturbed again and permanent seeding and mulch if areas have been final graded and accepted by
Engineer and owner.
1.) Slopes steeper than 3:1 and perimeter dikes, swales, ditches and slopes, and area new HQW waters -
within 7 days.
2.) Slopes 3.1 or flatter, and all other areas with slopes flatter than 4:1 - within 14 days. If temporary
seeding is used to temporary stabilize an area upon final grading of the job the permanent seed,
fertilizer and mulch shall be completed. See detail for seeding specifications.
2. Seeding Specifications:
Apply lime and fertilizer and work into 4 - 6 inches of soil. Seed mixture shall be distributed uniformly and
covered with a clean straw mulch. All mulch shall be crimped or asphalt tacked to hold in place. Material and
application rates as follows, and are generally considered minimums. "Mulch rate to be doubled if crimped*:

3. Contractor is responsible for complying with erasion control permit conditions and NPDES general permit
conditions. See permits for additional information.
4. Contractor shall clean out, amend, and maintain all measures as required to prevent accelerated erosion from
taking place on this site, until site is restabilized and accepted by, Engineer and NCDEQ.
5. Upon restabilization of all disturbed areas and swales, contractor shall remove all temporary erosion
control facilities and grade, gross, mulch, and restabilize the areas.
6. Once contractor has completed job and site is restabilized, the developer shall be responsible to maintain
the restabilized areas until time when the lots are sold or developed.
