Dear Mr. Bagnell:
Attached to this letter please find a copy of the Compliance Inspection Report Form for the
Compliance Inspection conducted on August 11, 2021 at the above referenced facility. The
Compliance Inspection was conducted by Randy Sipe with the Water Quality Regional
Operations Section Washington Regional Office. Mr. Junior Edwards, the ORC for the facility,
was also present and provided very valuable assistance during the inspection.
During the inspection all the wastewater treatment and disposal components of the facility
described in Permit No. WQ0034880 were toured. The inspection also included a review of
monthly reports/recordkeeping.
Overall the facility appears to be currently providing adequate treatment and disposal of
wastewater and is considered to be generally in compliance with Permit No. WQ0034880;
however, the following operational and maintenance issues were identified at the facility during
the inspection:
 The infiltration basins have significant woody vegetation surrounding them. While it
does not appear that operation of the basins is currently affected, continued presence of
this vegetation could affect the operation of the basins in the future as it seasonally dies
off and settles into the basins. Condition III.10 of Permit No. WQ0034880 requires that
embankments surrounding the basins be keep free of trees, shrubs and other woody
vegetation and that the embankments be kept mowed or otherwise controlled and
accessible.
