On May 31, 2013, the Division of Water Quality (Division) received your applications dated March 26, 2013,
requesting a 401 Water Quality Certification from the Division for your project. The Division has determined
that your applications are incomplete and cannot be processed. The applications are on -hold until all of the
following information is received:
1. Per Condition 6 of GC 3890, all work must be done "in the dry ". Therefore, dewatering methods will
be necessary to temporarily dewater the stream channels during pipe installation. Please provide
construction drawings /plans that clearly show the location /limits of the temporary impacts from
dewatering and provide revised impact tables which notes these temporary impact lengths.
2. Please clarify whether the Division of Energy, Mineral and Land Resources will require riprap within
any stream channels as part of the Erosion & Sedimentation Control Plan approval. If riprap is
required, please enumerate and clearly label the temporary and permanent impacts on the site plan
and submit a channel restoration detail. Riprap must be located below the stream bed elevation.
3. It appears that the StormTech chambers are being proposed to support an underground infiltration system.
This is an acceptable approach to managing the stormwater from this site. However, the Division will need
additional information to review the system and determine if state design standards have been met. Please
provide the following:

a. Full -sized plan sheets showing the entire site with its storm drainage system, including every inlet,
outlet and conveyance device. Please show how stormwater management requirements have been
met at every proposed outlet location.
b. Please provide an Infiltration System Supplement Form with all required items for the StormTech
system (see http: / /portal.ncdenr.org /web /wq /ws /su /bmp- manual).
c. Sealed, signed and dated supporting calculations for each BMP.
d. An original signed and notarized O &M Agreement form. Sample O &M forms are available at:
http: / /Portal.ncdenr.org /web /wq /ws /su /bmp- manual.
e. A soils report that states the level of the SHWT elevation based on a boring within the footprint of each
of the proposed BMPs.
Pursuant to Title 15A NCAC 02H .0502(e), the applicant shall furnish all of the above requested information
for the proper consideration of the application. If all of the requested information is not received in writing
within 30 calendar days of receipt of this letter, the Division will be unable to approve the application and it
will be returned. The return of this project will necessitate reapplication to the Division for approval,
including a complete application package and the appropriate fee.
Please respond in writing within 30 calendar days of receipt of this letter by sending two copies of all of the
above requested information to the Wetlands, Buffers, Stormwater — Compliance and Permitting (Webscape)
Unit, 1650 Mail Service Center, Raleigh, NC 27699 -1650.
Please be aware that you have no authorization under the Water Quality Certification Rules for this activity
and any work done within waters of the state may be a violation of North Carolina General Statutes and
Administrative Code.
