On June 17, 2013, the Division of Water Quality (Division) received your application dated June
6, 2013, requesting a 401 Water Quality Certification and Buffer Authorization from the Division
for your project We understand that you are requesting to modify the 401 Certification and
Buffer Authorization that were previously obtained Please understand that even though you
are modifying authorizations that were previously obtained, each request for a 401 Certification
and Buffer Authorization must stand on its own, and all of the requested information on the
application must be submitted The application is on -hold until the following information is
received
Please provide a site plan showing streams and protected riparian buffers as overlays on
the site plan Zone 1 and Zone 2 of the riparian buffer must be differentiated on this
plan
Enumerate all stream and riparian impacts on the site plan and clearly label impacts
These impacts must then correspond to the impacts listed in the application
3 Since this project proposes to impact protected riparian buffers, there needs to be a
diffuse flow plan or an explanation for why a diffuse flow plan is not included with the
application "See original application" is not an adequate response to the question

Pursuant to Title 15A NCAC 02H 0502(c) and Title 15A NCAC 02B 0233 (8), the applicant shall
furnish all of the above requested information for the proper consideration of the application
If all of the requested information is not received in writing within 30 calendar days of receipt
of this letter, the Division will be unable to approve the application and it will be returned The
return of this project will necessitate reapplication to the Division for approval, including a
complete application package and the appropriate fee
Please respond in writing within 30 calendar days of receipt of this letter by sending four copies
of all of the above requested Information to the Wetlands, Buffers, Stormwater — Compliance
and Permitting (Webscape) Unit, 1650 Mall Service Center, Raleigh, NC 27699 -1650

Tar - Pamlico Riparian Buffer Rules for this activity
