Pursuant to G.S. 143-215.6A.(f), the Division of Water Resources has reviewed your remission request and
supporting documentation received July 22, 2021, and have found justification to modify the initial civil
penalty assessment of $2,000.00 plus $140.56 in enforcement costs to a revised civil penalty assessment of
$1,800.00 plus $140.56 in enforcement costs.
Within 30 days of receiving this notice, you shall do one of the following two options:
1. Submit civil penalty assessment payment:
Payment shall be made in the form of a check, money order, or electronic funds transfer made payable
to the North Carolina Department of Environmental Quality in the amount of $1,940.56. Payment
resolves this civil penalty; however, it does not prevent further enforcement actions for continuing or
new violations. Please submit payment to the following address:

If payment is not received within 30 days of receiving this letter, your civil penalty remission request
and the Division of Water Resources' recommendation to deny the request will be delivered to the

Remissions (Committee) for final agency decision pursuant to G.S. 143-215.6A.(f).

If you desire to make an oral presentation to the Committee on why your civil penalty remission request
meets one or more of the statutory factors, please complete and return the attached form within 30 days
of receiving this letter. All documentation shall be submitted to:

The EMC Chairman will review your oral presentation request and supporting documentation to
