Dear Mr. Holloman,
The Division of Water Quality(DWQ)received a Public Notice issued by the US Army Corps of Engineers
on May 1,2013(SAW-2013-00404).An Individual 404 Permit will be required for this project,which also
requires an Individual 401 Water Quality Certification. To request a 401 Water Quality Certification,
please submit the following:
1. Application Package: submit five(5)complete sets of the application(Eng Form 4345,Oct 2012)
and associated maps to the DWQ Central Office in Raleigh(1650 Mail Service Center, Raleigh NC
27699-1650).The 401 Certification cannot be processed until five(5)complete sets of the
application and associated maps are received at the DWQ Central Office in Raleigh along with
the appropriate fee.
Any large scale maps that are provided also need to include a copy of the site plans on a cd.One
(1)data CD of full size plans in TIFF Group 4 format(black and white, not grayscale or color). If
the plans are too large to store in TIFF format,they can be stored in PDF. If you have questions
pertaining to this, please call Bev Strickland at(919)807-6350.
2. Application Fee:The fee for 401 Water Quality Certification is$240 for projects impacting less
than an acre of wetland and less than 150 linear feet of streams(whether intermittent or
perennial). For projects impacting one or more acres of wetland or 150 linear feet of streams
(whether intermittent or perennial)or more,the fee is$570.
Until the information requested in this letter is provided, DWQ does not have an application for a 401
Water Quality Certification.
