The Division has reviewed your application, received on September 21, 2020, for renewal of NPDES
permit NC0087891. To enable us to complete our review in accordance with N.C.G.S. 143-215.1 and
15A NCAC 02H .0105, we need additional or revised information to address the following comments:

• EPA Form 1 for municipal POTWs (refence only), and Modified EPA Form 2A for municipal
POTWs with a design flow less than 0.1 MGD. These forms can be downloaded at the following

We cannot continue processing the permit renewal for South Toe River WWTP until we have received
the additional information requested above. Please submit the additional information requested by one
