Date: 8/10/2021 On August 4, 2021, staff visited the site to review the permit rescission request. The Hampstead Pines subdivision has tied on to the Pluris Hampstead facility (WQ0037287). Their sewer system permit (WQ0023479) was transferred to Pluris Hampstead on May 1, 2020. Their last filed effluent report from June 2020 says flow ceased from it on June 8, 2020. All wastewater and residuals have been removed and the wastewater facility has been demolished, the 5-day upset pond and reclaimed water storage pond have been decommissioned, including removal of the synthetic liners and grading of the site. All monitoring wells have been properly abandoned with well abandonment records submitted. Prior groundwater monitoring data indicated consistent compliance for many years prior to abandonment. The former WWTF site is now under development with single family homes currently under construction. The WiRO recommends permit recission. FORM: WQROSNDPRR 08-17 Page 1 of 1
