On 7/15/2021, Ron Boone and Tricia Lowery met with Brad Flynt (ORC), Elijah Williams, and Alicia
Goots (Laboratory) to perform a Compliance Evaluation Inspection at TZ Osborne WWTP. This
inspection consisted of 2 parts: 1) in -office file review and, 2) on -site inspection of the treatment
facility. Ms. Goots assisted us with the in -office file review and Mr. Flynt escorted us through the
facility during the on -site inspection. The attached EPA inspection form details the areas that were
evaluated with their respective comments (if needed).
The facility runs and operates in accordance with their permit. No significant issues or violations were
found during the inspection.

Comment: Permit expired June 30, 2019. Application for renewal was received 11/26/18.
Equalizations basins are not listed in the permit under treatment components. Special
conditions exist until upgrade to 56 MGD for nutrient limitation, loading and allocations,
as well as, MMP, NOP, chronic aqtx, and efflluent pollutant scan. SOC became
effective 5/1/2021 for weekly sampling for 1, 4-dioxane using EPA approved method
624.1. Plant upgrade from 40MGD to 56MGD is expected to occur 8/1/2021, but
possibly 9/1/21 at the latest.

Comment: Old EQ basin 3.5 MG capacity, new EQ basin 4.5 MG capacity. EQ's are fed from grit
chamber. If critical threshold flow events (96mgd) were to occur, the 2 EQ basin will
bypass EQ pump station and directy enter BNR to prevent flooding/overflow of pump
station.
