Sediment was not lost beyond the turbidity curtain. Degree of damage: Slight 8. Corrective action needed: Please follow the guidance provided by Ryan Conchilla, and address site improvements as outlined below. 10. Comments: This inspection occurred as a result of a self -reported sediment loss; the loss was reported on July 20, 2021, to Ryan Conchilla, Environmental Specialist II, 401 and Buffer Transportation Permitting Branch (Divisions 7 & 8), NCDEQ DWR, by Jerry Parker, NCDOT Division 7 Environmental Supervisor. Site pictures were provided. Mr. Conchilla reported the loss to DEMLR the following day, July 21, 2021. It was determined that the site ESC measures in place held during the strong rain event preceding the release into the on -site stream. The turbidity curtain was anchored properly and prevented impact beyond the curtain. During the inspection, the following maintenance recommendations were discussed and requests for additional ESC measures were made: 1) The wattle at the culvert pipe inlet located between basins 15.1 and 15.4 (the wattle is physically located in the stream in front of the culvert) needs to be cross -staked and secured. 2) The silt fence to the right of the culvert (facing North) needs to be pulled up and secured. 3) The coir fiber matting along the stream bank to the right of the culvert (facing North) needs to be cross -staked and secured. 4) The geotextile fabric on the emergency spillway of basin 15.1 needs to be secured and pinned down. The wattles located at the end of the spillway need to be cross -staked and secured. 5) Install additional wattles to the right of basin 15.1 (facing West) to help prevent the overflow from bypassing construction stormwater control. Hanes Mill Road, Suite 300, Winston-Salem, NC 27105 (336) 776-9800 6) Sediment needs to be removed from bay 1 and 2 of basin 27.1. It was recommended this be done during conversion of this basin to a Hazardous Spill Basin. Note: Sediment located within the culvert pipe flows throughout the length of the pipe. An estimated area 2' x 4'x 4" was observed at the inlet end and a much larger area that was unable to be estimated was observed at the outlet end. The method of removal is being evaluated by DOT and Mr. Conchilla. Please contact this office as maintenance items are completed. Photo logs of the maintenance performed and enhanced ESC measures would be appreciated.
