In accordance with your request letter received August 19, 1983, we are
forwarding herewith Permit No., 3797-R3, dated October 25, 1983, to Holiday Island
Property Owners Association, Inc. for the continued operation of the subject
non -discharge type wastewater treatment and disposal facilities.
This permit shall be effective from the date of issuance until October 31,
1986, shall be subject to the conditions and limitations as specified therein,
and shall void Permit No. 3797-R2, issued June 9, 1983.
If any parts, requirements, or limitations contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing before a
hearing officer upon written demand to the Director within thirty (30) days
following receipt of this permit, identifying the specific issues to be contended.
Unless such demand is made, this permit shall be final and binding.

For the Discharge of Sewage, Industrial Wastes, or Other Wastes
In accordance with the provisions of Article 21 of Chapter 143, General
Statutes of North Carolina as amended, and other applicable Laws, Rules, and
Regulations
PERMISSION IS HEREBY GRANTED TO

FOR THE
continued operation of the existing collection system, influent pump station, 20,000
GPD extendedaeration wastewater treatment plant with sludge holding and chlorination
facilities, a 10,000 gallon aerated holding tank, dual tertiary filters, dual effluent
discharge pumps with high-water alarm and approximately 2,700 lineal feet of 4-inch
force main and a surface/overland effluent disposal system consisting of 1,004 lineal
feet of 4-inch drain tile in gravel filled trenches and the construction and operation
of approximately 300 lineal feet of additional 4-inch drain tile in gravel filled
trenches to serve Holiday Island Property Owners Association, Inc. such that there is
no contamination of the groundwaters nor any discharge to the surface waters of this
State,
pursuant to the request letter received August 19, 1983, and in conformity with the
plans, specifications, application and other supporting data, all of which are filed
with the Department of Natural Resources and Community Development and considered a
part of this permit.
This permit shall be effective from the date of issuance until October 31, 1986,
shall be subject to the following specified conditions and limitations, and shall void
Permit No. 3797-R2, issued June 9, 1983.
1. This permit is effective only with respect to the nature and volume of wastes
described in the application and other supporting data.
2. The facilities shall be effectively maintained and operated at all times so
that there is not any discharge to the surface waters, and any contamination of
the groundwaters which will render them unsatisfactory for normal use. In event,
the facilities fail to perform satisfactorily, including nuisance conditions,

failure of the disposal area to adequately treat the wastewater, the Property
Owners Association shall take such immediate corrective action as may be
required by this office.
3. Upon the availability of a regional sewerage system, the Holiday Island
Property Owners Association, Inc.'s wastewater treatment facilities shall be
abandoned, and all wastewater discharged into the regional system.
4. The chlorination facility shall be properly operated at all times to produce
a continuous chlorin; residual of not less than 0.5 mg/1 in the plant effluent
after 30 minutes contact time at average daily flows.
5. This permit is not transferable.
6. The wastewater treatment facilities shall have units furnished, arranged and
connected in such manner that biological treatment with final clarification,
tertiary treatment and effluent disinfection are provided at all times, including
during periods when site is flooded by surface waters.
7. All sludge from these facilities, disposed of by any means other than a
commercial septic tank pumper, shall be permitted by the Division of Environmental
Management in accordance with G.S. 143-215.1.
8. A monitoring program shall be implemented as follows:

Note: All samples shall be effluent grab samples obtained at the effluent
pumping station.
9. There shall not be any sewer connection or other sources of wastewater
discharge made, which results in the capacity of the existing 20,000 GPD
tertiary type treatment plant being exceeded.
10. Bypassing of any of the facilities which will resulting a plant discharge of
inadequately treated wastewater is prohibited, and such action shall be cause
for closing part or all of the Holiday Island development and/or revoking
this permit.
11. Monitoring wells shall be constructed and sampling schedules arranged as
required by the Division of Environmental Managementand constructed in
accordance with 10 NCAC 2C 0.0100.

12. Any monitoring deemed necessary by the Division of Environmental Management to
insure surface and groundwater protection will be established and an acceptable
sampling reporting schedule shall be followed.
13. Self -monitoring reports on the operations of the facilities shall be submitted
to the Division's Washington Regional Office within 45 days following the end
of the reporting period (monthly) and in such form and detail as may be required
by this Division.
14. The Permittee, at least three (3) months prior to the expiration of this permit,
shall request its extension. Upon receipt of the request, the Commission will
review the adequacy of the facilities described therein, and if indicated, will
extend the permit for such period of time and under such conditions and limitations
as it may deem appropriate.
15. The Permittee shall employ a certified wastewater treatment plant operator in
responsible charge of the wastewater treatment facilities. Such operator must
hold a certificate of the grade at least equivalent to the classification assigned
to the wastewater treatment facilities by the Certification Commission.
16. The issuance of this permit shall not relieve Holiday Island Property Owners
Association, Inc. of the responsibility for damages to surface or groundwaters
resulting from the operation of this facility.
17. Adequate inspection, maintenance, and cleaning shall be provided by the Permittee
to insure proper operation of the subject facilities.
