The following is a brief summary of the discussions on the project:
Introductions were made by Herb Turner (Mott MacDonald) and all in attendance. Herb proceeded to give an
overview of the project (I-5987A & B).
· For I-5987A, David Bocker (NV5) and Bill Jernigan (MI Engineering) proceeded to review Section A1
and A2, respectively. Refer to meeting minutes for I-5987A (under separate cover).
For I-5987B, the meeting resumed at 11:35am. Herb Turner introduced the hydraulic Design Team for I-
5987B and Kevin Alford (Wetherill Engineering) proceeded with sheet-by-sheet review of the permit drawings:
Stormwater Management Plans:
· Robert Patterson noted that standard rip rap pads do not meet the definition of the Riprap Energy
Dissipator Basins on Sheet 3 of the Stormwater Management Plan (SMP) and should therefore be
removed from the SMP.

· Robert Patterson also suggested the possibility of adding grass swales to the SMP. David noted that
treatment is not needed so grass swales were not included. Paul Atkinson added that if not required,
grass swales are typically not shown. Paul was going to double-check with the Stormwater staff at
NCDOT and follow-up with the Design team. After the meeting, Paul directed the design team per the
Stormwater group to only call out grass swales used for treatment (where treatment is required) on the
swale tab. A note has been added to the general project narrative that vegetated conveyances were
used throughout the project which will have water quality benefit.

o Liz Hair asked if there were any wetland impacts due to updated jurisdictional determinations.
Kevin said the design team will go back and verify. After the meeting it was verified that a
wetland file provided on 6-16-21 showed wetlands that will be impacted by the project at this
site. These wetlands did not show up in the previous files.

Plan Sheet 10/11/12:

o Jim Rerko mentioned that a workpad will be needed under the bridge. Kevin Alford and Herb
Turner confirmed that the workpad was accounted for but it will not show up in the profile view
because of the location that the profile was cut at.
o Paul Atkinson wanted to make sure the workpad was labeled in plan view and Kevin Alford
verified that it was labeled.

o Herb Turner mentioned that after the last non-merger meeting, the downstream end of the
proposed 2 @ 54” was re-evaluated. It was found to be a tributary as well. The outfall
cleanout was reduced to tie into the existing 24” RCP (on Plan sheet 13A). From the 2@54”
to the outlet of the 24” RCP is JS non-mitigable. The JS mitigable determination will start at
the outlet of the 24” RCP.

o Excavation impacts shown on the plans are required for a bore pit to install the 24” by
trenchless installation. Paul Atkinson wanted to know if the excavation of the pit will be shown
as temporary impacts or permanent. Paul wanted to make sure the impacts were shown
correctly since it is shown as permanent. Liz Hair wants a note added to the impact summary
table describing the impact. The note should state that the excavation area will be filled in
after the pipe is installed, the impacts should only last about a 1 month +/- and the site does
not require mitigation. Deanna Riffey stated that on the I-540 project that have both
excavation and fill impacts shown. Liz stated that it this site does not need mitigation, since it
will be temporary excavation that will be filled back in and restored. Mechanized clearing will
be shown as it is currently shown. It was decided there will be no changes to the way the
impacts are shown and a note will be added to the impact summary sheet describing the
impacts.
Plan Sheet 18:
· Site 9
o The detail callout for the inlet and outlet of the culverts does not match a detail on the detail
sheet. Verify that the callout is correct or if the detail needs to be added. After the meeting it
was found that the callout on the plans was correct, but the detail shown in the ditch details did
not had not been labeled. A label was added to the details to match the plans.

Plan Sheet 28/29/30:

o The bridge is currently being re-designed to be a 2 span, rather than a 3 span. Paul Atkinson
wanted to verify that the correct design is shown in the permit drawings, since a 2 span bridge
is shown. Herb Turner confirmed that it is shown correctly.
Plan Sheet 37:
· Site 13
o Paul Atkinson said he assumed the culvert was not buried because it is perched. Herb Turner
confirmed. Josh Dalton stated it was not buried because the channel is higher on both ends
and burying it would further put the culvert in a hole.

o No mitigation is required for rip rap fill in the existing stream bed to bring the bed elevation
back to the pre-scour elevation. Rip rap in the channel will need a separate line item on the
impact summary since it will not require mitigation. A note will be added to the impact
summary.
Additional Discussion Topics
· Jim Rerko reminded the team that there will be a quick turnaround after this meeting. The permit
drawings will need to be complete in a week or two. NV5 will be doing the permit application and will
need to have it submitted by mid-August.
· Paul Atkinson followed up after the meeting in regards to showing grass swale information in the
stormwater management plan. His email stated that it has been confirmed with the NCDOT
Stormwater group that we only call out those grass swales specifically used for treatment (where
treatment is required) on the swale tab as they have specific post-construction inspection and
maintenance requirements. It should still be noted in the general project narrative that vegetated
conveyances were used throughout the project which have a water quality benefit”
