Division of Water Resources' Central and Regional staff has reviewed the application package
received June 16, 2021. However, additional information is required before the review may be completed.
Please address the items on the attached pages no later than the close of business on September 3, 2021.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A and B, or failure to provide the additional information on or before the above requested date
may result in your application being returned as incomplete.
Please reference the subject application number when providing the requested information. All
revised and/or additional documentation shall be signed, sealed and dated (where needed), with one
electronic copy submitted to my attention at the address below.

A. Application:
1. The existing Permittee's signature is required in accordance with 15A NCAC 02 T .0106(b). Please
have Mr. O'Briant or another delegated representative sign Form NDPONC 06-16.
2. The application has not been signed on behalf of the new Permittee by a signature authority.
Pursuant to 15A NCAC 02T .0106(b)(4) the permit application must be signed by either an
executive officer, elected official in the highest level of elected office, or other authorized
employee. Mr. Sterling D. Baker is an engineer and does not meet this requirement. Please have an
authorized person or a delegate sign Form NDPONC 06-16.
3. Delegation of authority to sign permit applications to other authorized employees or any employee
in a specific position may be provided in writing to the Division and signed by an authorized person.
B. Property Ownership Documentation:
1. Supporting information for Property Ownership has not been provided. Please provide a copy of
legal ownership (i.e. GIS or Deed).
