A review of this report was conducted by Division staff person Patrick Mitchell. Review of the subject annual report reflects compliance with Permit No. WQ0003796. However, annual certification requires that an explanation be provided in cases where the report shows no land application activities. Please provide a written explanation for residuals handling, storage or disposal in the absence of land application within ten (10) days of receipt of this letter. As a reminder, Permit Condition III. 3. requires that the Winston-Salem Regional Office (WSRO) shall be notified at least 24 hours in advance of any land application event. Please ensure that the WSRO is notified prior to the next land application event. An announced routine facility compliance inspection is planned to be scheduled within the next 12 months.
