I'm working the 6-Step Process for the Stormwater eDMR and wanted to reach out to you in regards to a few minor
updates our Stormwater Permit Summary Report.
On Step 1-
- The Owner Verification Report, the Owner Address zip code should be 28625 instead of 28677
- The Stormwater Permit Summary Report:
o Facility Name — no change
o Facility Address — as stated above, zip code is 28625
o Owner Name — no change
o Owner Affiliation Name — no personnel change, only need to update personnel email address to
[e-mail]
o Owner Affiliation Address — same as above, zip code is 28625
o Owner Contact Person — no change
o Facility Contact Person — no change
o Permit Contact Person — no change
o Permit Billing Contact— no change
o Person with Delegated Signature Authority— I'm attaching our Stormwater Permit Delegation of
Signature Authority Form to add another delegated person.
o Outfalls — no change
