Brian,
I was looking over the additional information you provided. Everything looks good except for a few small issues:
Please correct the following:
- Part D1 – Steven McLendon is listed as the signing official, however Part G of the Application is signed by David
Kale. If you would like to change the signing official to David Kale, please let me know (I can write this change in
on the original Applications that you provided so you don’t need to resubmit revised hard copies of the
Application). Under normal circumstances, we would still require some official documentation stating that
signing official (David Kale) is the Board Chairman of Wesley Chapel VFD (Original Comment #8), however, the
signature on the Application was notarized by Andrew Stallings (who is listed as the registered agent for Wesley
Chapel VFD on the SoS website), so we will accept that as meeting the requirement. If you would like to keep
Steven McLendon as the signing official, you will need to provide 2x signed hard copies of the Application (with
Steven McLendon signing Part G).

upload, please let me know so that I may scan and upload one of the hard copies that I have).

Since these are really minor things, once I get the requested information and the documents are uploaded to Laserfiche
(Provided everything is still in order), we can issue the permit without doing another formal 30-45 day review (I’m
currently drafting the permit so we will be able to issue this permit very quickly once we receive the requested items).
