Snider, Regional Supervisor North Carolina Department of Environmental Quality Division of Water Resources 450 West Hanes Mill Road, Suite 300 Winston-Salem, North Carolina 27105 NOV2021DV0285 Re: Sanitary Sewer Overflow Civil Penalties Incident #202101034 Dear Mr. Snider: This letter serves as the official response from Winston-Salem/Forsyth County Utilities (WSFC Utilities) to the North Carolina Department of Environmental Quality, Division of Water Resources Notice of Intent to Enforce Civil Penalty for the March 15, 2021 sanitary sewer overflow (SSO), Incident #202101034, located at 5288 Ivystone Lane in Winston-Salem. The incident was a direct result of pipe failure. On March 15, 2021 at 12:00pm, an SSO occurred at 5288 Ivystone Lane. Our outfall clearing crew discovered a manhole had fallen into the creek due to severe erosion. They stopped the flow at an upstream manhole by plugging the outgoing pipes and setting up a pump and haul at two locations until repairs were made. The response to this incident demonstrates our ability to pivot quickly to address critical improvements, as well as our commitment to investment in infrastructure.
