Mr. Daniel Boss,
We received a notice of violation (NOV-2021-LV-0483) for the Midway Medical Facility
(NC0022454), June 30, 2021 in regards to the May 2021 DMR for 001 Effluent Ammonia
discharge which was collected on 5/31/2021. The reason for this letter is to provide explanation
of why the limit of 6 was exceeded and the corrective action conducted. During the month of
May the complex, in which the facility is located, was having both interior and exterior
renovations completed. Within the time frame of the renovations an unknown substance was
received into the wastewater treatment plant. The introduction of the substance killed healthy
bacteria in the treatment system, which in turn resulted in an ammonia level higher than what is
prescribed in the permit.
Once aware of the exceedance we took several corrective actions. Nutrient rich dog food
was added to the aeration basin to aide in the rebound of healthy bacteria, and the pH was
increased to assist in the nitrogen break down cycle of ammonia. Aluminum sulfate was added to
the treatment facility to aide in precipitating out the unknown substance. Once corrective action
was taken the facility quickly regained compliance and has maintained compliance to date.
