We have an intersection improvement project here in Division 5 that requires stream and wetland mitigation. I have
attached a completed form for the project that hopefully provides you with the necessary information for this
request. Can you please send and acceptance letter for this mitigation request?

The big issue here is that any/all charges for the mitigation should be directed to this WBS number: 50138.1.155

The purpose of this letter is to notify you that the Division of Mitigation Services (DMS)will provide the
compensatory stream and wetland mitigation for the subject project. Based on the information supplied by you on
July 23,2021,the impacts are located in CU 03020201 of the Neuse River basin in the Central Piedmont(CP) Eco-
Region,and are as follows:

Lieu Fee Instrument dated July 28,2010. If the above referenced impact amounts are revised,then this mitigation
acceptance letter will no longer be valid and a new mitigation acceptance letter will be required from DMS.
