Enclosed is the DMR for outfalls SW001, SW002, SW003, SW0048, SW056, SW073, SW075, SW077, SW078, SW079, SW080 and SW081. There were no samples collected from these outfalls and are listed as "No Flow" for the sampling period. This was due to either nighttime rainfall events, inclement weather or lack of qualifying rainfall events. C o O '`.a' .G O u oz ti U In Select Language NORTH CAROLINA Environmental Quality NC DEQ NPDES Industrial Program The NPDES (National Pollutant Discharge Elimination System) Industrial Stormwater Program is federally mandated and covers a wide variety of industrial actvities (htq)s://files.nc.gov/ncdeq/Energy%2OMineral%20and%2OLand%2OResources/Stormwater/NPDES_Gencral Guidance/NPDES-SW-11Categories-20160127- DEMLR-SW.pd fl under one of the following types of permits: • Individual Industrial Permits (hps://deq.nc.gov/about/divisions/energy-mineral-land-resources/npdes-stormwater- individual) apply to industrial activities that are not eligible for any of the general permits. (approx. 160 facilities in NO • General Industrial Permits (hops deq.nc.gov/about/divisions/energy-mineral-land-resources/npdes-stormwater-gps) apply to numerous broad categories of industrial activities with potential stormwater discharges. (approx. 3,200 facilities in NO • No Exposure Certifications (hh ps: deq nc.gov/about/divisions/energy-mineral-land-resources/npdes-no-exposure) apply to industrial activites that do not expose industrial materials or activities to precipitation, and provide secondary containment on the site. How to Submit Paper DMRs Before eDMR is Implemented The Stormwater Program is planning for NPDES Stormwater Industrial permitted facilities to use electronic Data Monitoring Reports by January 1, 2022. Until your facility begins using eDMR, follow this NEW procedure for submitting eDMRs. • Download the applicable DMR from the NPDES Stormwater Industrial Individual website (hftps deq nc.gov/about/divisions/energy-mineral-land-resources/npdes-stormwater-individual) or the NPDES Stormwater General Permit website (hops: deq.nc.gov/about/divisions/energy-mineral-land-resources/npdes-stormwater-gps). • Complete, sign, scan and upload the DMR to the NPDES Stormwater Permit Data Monitoring Report (DMR) Upload form fhftps:lledocs.deg.nc.gov/Forms/SW-DMR) within 30 days of receiving sampling results. • Mail the original, signed hard copy of the DMR to the appropriate DEMLR Regional Office (hit ppadLd-gq.nc.gov/contact/regional_ off ices). • Do not send DMRs to Central Files in Raleigh! (This is a change from the previous procedure.) • Note that DMR forms have recently been updated. How to Pay an Annual Permit Fee Invoices are sent when fees are due per the NPDES Stormwater Permit Fee Schedule (https://files.nc.gov/ncdeq/Energy%2OMineral%20and%2OLand%2OResources/Stormwater/NPDES%201ndusMal%20S W %2OPennit%2OFees%2020170310.pdf) Electronic payment of annual permit fees is preferred. Click on "Stormwater ePayments" on the lower right corner of this page. There is no fee for eCheck. The convenience fee for a credit card is 2.65% and the convenience fee for a debit card is $ 3.95. Who Can Sign NPDES Industrial Permit Documents? 40 CFR 122.22(bttps://www.govregs.com/regulations/expand/title40 cbapterI-i23 vartl22_subpartB_sectionl22.22#regulation_1) sets forth who can sign NPDES applications and reports. For corporations, permit documents must be signed by responsible corporate officer, and authorized manager, or a delegated individual. An authorized person may delegate an individual using the Stormwater Delegation of Signature Authority Form (https://files.nc.gov/ncdeq/Energy%2OMineral%20and%2OLand%2OResourccs/Stormwater/NPDES%2OMiscellaneous%2OFonns/S W-DOSA-Form- 20210202.pdfl After completing the Stormwater Delegation of Signature Authority Form (https://files.nc.gov/ncdeq/Energy%2OMineral%20and%2OLand%2OResources/Stormwater/NPDES%2OMiscellaneous%2OFonns/S W-DOSA-Form- 20210202.pdfl , scan and email it to Suzanne McCoy (maitto:suzanne.mccoy@ncdenr gov) and mail the original, hard copy to the Stormwater Program, attn: Suzanne McCoy, 1612 MSC, Raleigh, NC 276990-1612. How to Update Permit Data Enter your stormwater permit number into the Stormwater Permit Summary Report (https://reports.ncdenr. org/BOE/0penDocument/2002150408/CrystalReports/viewrpt.cwr? id=585684&apsusei=BIMS_U&apspassword=b 10Ms_cr%28ddnri59%29&apsauthtype=secEnterprise&cmd=EXPORT&EXPORT_FMT=U2FPDF%3A0) and you will see a form that includes the address of the facility, contact information for the permit, and the regulated stormwater outfall(s) for the facility. To update the: • Facility name or ownership, complete the Name/Ownership Change Form (hit ps://edocs.deq,nc.gov/Forms/SW-Name- Change). • Legally responsible person, complete the Owner Designation/Affiliation Form (h ps:lledocs.deg.nc.gov/Forms/SW-Owner- AffiLiation), • Facility address or contact information, email Suzanne McCoy _(mailto:suzanne.mccoy@ncdenr.gov). • Billing contact, email Aziza Selkane (mailto.aziza.selkane@ncdenr.gov). • Stormwater outfaLl information, email Suzanne McCoy_(mailto:suzanne.mccoy@nc enr.gov). Request Representative Outfall Status Complete the Representative Outfall Status (ROS) Request Form (https://files.nc.gov/ncdeq/Energy%2OMineral%20and%2OLand%2OResources/Stormwater/NPDES_Fillable_PDF Forms/NPDES-ROS%2ORequest- 20171026-DEMLR-S W.p df) . Mail one hard copy of the form and supporting documents directly to the appropriate Regional Office (see form). In addition, e- mail the form and supporting documents to Suzanne McCoy_(mailto:suzanne.mccoy@ncdenr.gov). There is no fee for a representative outfall status request. Renew an Existing Industrial Permit For Individual permits, complete the NCS Renewal Forms Package (https:Hfiles.nc.gov/ncdeq/Energy%2OMineral"/o20and%2OLand%2OResources/Stormwater/NPDES%201ndividual%2OPennits/NPDES-NCSRenewal-Form- Package-20131018-DEMLR-S W.pdfl , no later than 180 days prior to expiration of your current permit. Mail one hard copy of the form and supporting documents to
