Stormwater Permit No. SW5200201
Lee Street Athletic Complex Improvements
Johnston County
Dear Mr. Shipman:
The Division of Energy, Mineral and Land Resources (DEMLR), received a Stormwater
Management Permit Application for the subject project on February 5, 2020. A review of that
information has determined that the application is not complete. The following information is
needed to continue the stormwater review:
As designed, the project does not meet the definition of low density (Section II, 2). One
of the tenets of low density is the use of piped conveyance only when unavoidable. It
appears that proposed Yard Inlet 200 and the 83 LF of 15" HDPE connecting it to the
existing system may be unnecessary. Please remove these unessential non -vegetated
conveyances and re -grade this area so that surface flow is directed to the existing
collection system or provide the information needed for a high density project.
2. Please provide an original, signed hard copy of the Supplement-EZ Form and O&M
Agreement Form (Section V & Section VI, 3). Both forms can be found at the following

land-permit-guidance/stormwater-bmp-manual NOTE: Both forms use macros for
customization, ensure that the forms are "enabled" and all of the "load forms" and "click
to update" links are clicked. The Supplement-EZ Form should have 3 pages (Cover sheet,
Drainage Areas sheet, and Low Density sheet). We are currently working on including a
low density option for the O&M Agreement Form. For the time being, please enter 1
"grassed swale" (As the maintenance to it is nearly identical) and make a note on the
form that this is a low density O&M Agreement or provide an acceptable non-standard
O&M Agreement.
3. Please include the bearings and distances for the property boundary on the plans (Section
VI, 8i).

4. Provide pdfs of all revisions, 2 hardcopies of revised plan sheets, and 1 hardcopy of other
documents. Pdfs must be uploaded using the form at:

Please note that this request for additional information is in response to a preliminary review.
The requested information should be received by this Office prior to April 15, 2020, or the
application will be returned as incomplete. The return of a project will necessitate resubmittal
of all required items, including the application fee. Please reference the State assigned project
number SW5200201 on all correspondence.
If you need additional time to submit the information, please submit your request for a time
extension to the Division at the contact below. The request must indicate the date by which you
expect to submit the required information. The Division is allowed 90 days from the receipt of
a completed application to issue the permit.
The construction of any impervious surfaces, other than a construction entrance under an
approved Sedimentation Erosion Control Plan, is a violation of NCGS 143-215.1 and is subject
to enforcement action pursuant to NCGS 143-215.6A.
