No. U-4009: $200.00 Debit work order 9.8059077, WBS 35010.1.1 REFERENCE: 1) USACE 404 NW 14 Permit, Action ID Number 200021007, dated November 16, 2006 2) NCDWQ 401 Water Quality Certification (WQC 3404), dated June 1, 2006, Project No. 20040090 v.2 Dear Sir: On June 1, 2006 and November 16, 2006, the United States Army Corps of Engineers (USACE) and North Carolina Division of Water Quality (DWQ) re-authorized the relocation of SR 1126 and the addition of a left turn lane onto Garret Road along with an additional westbound receiving lane on Garret Road via a 404 NW 14, (Number 200021007), and a 401 Water Quality Certification (WQC 3404). The permits expired on March 18, 2007. Table 1. SUITE 240 RALEIGH NC 27604 As indicated in the table, total stream impacts are 45 feet and wetland impacts are 0.075 acres. The project at this time has a Let Date of July 17, 2007. NCDOT respectfully requests that the referenced section 404 NW 14 and section 401 WQC be re-issued. The project has not changed. Therefore, all previously submitted permit drawings and plan sheets are current. All environmental commitments and special conditions, referenced in the NW 14 Permit Application, will continue to be followed throughout project construction.
