Failure to submit all required items will necessitate additional processing and review time. For more information, visit the System -wide Collection System Permitting- website: General — When submitting an application to the Pretreatment, Emergency Response, & Collection Systems (PERCS) Unit, please use the following instructions as a checklist in order to ensure all required items are submitted. Adherence to these instructions and checking the provided boxes will help produce a quicker review time and reduce the amount of requested additional information. The Applicant shall submit one original and one conv of the application and supporting documentation. A. Cover Letter El Submit a cover letter listing all items and attachments included in the permit application package B. No Application Fee Required > No application fee is necessary. The permittee will be billed an annual fee upon issuance of the permit ➢ The appropriate annual fee for systemwide wastewater collection system permits may be found at: > Annual Non -Discharge Fees C. Any unauthorized content changes to this form shall result in the application package being returned. If necessary for clarity or due to space restrictions, attachments to the application may be made, as long as the attachments are numbered to correspond to the section and item to which they refer. ❑ If the Applicant Type in Section I.3 is a Privately -Owned Public Utility, provide the Certificate of Public Convenience and Necessity (CPCN) from the North Carolina Utilities Commission demonstrating the Applicant is authorized to hold the utility franchise for the area to be served by the wastewater collection system, or ❑ Provide a letter from the North Carolina Utilities Commission's Water and Sewer Division Public Staff stating an application for a franchise has been received and that the service area is contiguous to an existing franchised area or that franchise approval is expected. ❑ If the Applicant Type in Section I.3 is a corporation or company, provide documentation if it is registered for business with the North Carolina Secretary of State. The Univeristy Physical Plant Plumbing department maintains and operates the collection system. work performed includes daily inspections of lift stations, round the clock moinitoring of alarms, monthly preventative maintenace and semi annual cleaning. 2. Purpose To outline a plan that will outline roles and responsibilities of UNCW departments in the event of a Sanitary Sewer Overflow with the mission of protecting the health of the UNCW community and the environment. II. Definitions Sanitary sewer overflow — is any overflow or release of sanitary sewage from the collection system location on the UNCW property. This includes but is not limited to the piping, plumbing fixtures, lift stations and manholes. De minimus SSO — Any SSO that is of an estimated quantity of less than 50 gallons and presents no material risk to the community health or the environment. Included in this are routine toilet overflows and sanitary sewer backups that affect one room or less and do not affect any living spaces. A reportable SSO is a SSO grater then 1,000 gallons to the ground or a SSO of any amount that reaches surface water (including through ditches, storm drains, etc.) III. Responsibilities Physical Plant — Maintain the sanitary sewer collection system and serve as primary response agency to SSO's Environmental Health and Safety (EH&S) — Server as technical advisors in the protection of public health and the environment and reporting agency. Procedures In the event of a de minimus SSO, Physical Plant shall stop the release, restore the system and sanitize the area, utilizing prudent methods including OSHA blood borne pathogens protocols if Applicable. These releases can normally be addressed within house resources. Physical Plant shall prepare Standard Operating Procedures (SOP's) outlining sanitation procedures If the SSO appears to exceed the De minmus levels but does not exceed a reportable quantity, Physical Plant shall notify EH&S of the release and coordinate with them on the containment, sanitation and reporting. These releases typical require outside resources such as vacuum trucks or additional sanitizing agents. EH&S reserves the option to notify the North Carolina Department of Environment and Natural Resources (NCDENR) Division of Water Quality (DWQ) during normal working hours. In the unlikely event that a reportable SSO occurs, Physical Plant shall notify EH&S of the release and coordinate with them on containment, sanitation and reporting. EH&S shall have the option of activating the Emergency Operation Center if necessary. EH&S shall notify the NCDENR DWQ during normal working hours. Messages or faxes shall not be used. A press release shall be drafted by Physical Plant and EH&S and released by University Relations not more than 48 hours following a reportable SSO. Physical Plant should retain a list of suitable contractors that may be used to assist in such extensive clean-ups. EH&S shall maintain a list of acceptable clean-up methodologies. Within five working days of the verbal report, EH&S shall submit a complete Collection System Sanitary Sewer Overflow Reporting Form to the DWQ using the SSO 5-day reporting form. Part I of the collection System Sanitary Sewer Overflow reporting form provided required information to the DWQ. Part II provides a justification for the spill. Records of all reportable and other SSO's deemed to be significant by Physical Plant or EH&S shall be maintained for a period of at least 3 years. Health & Safety TITLE: Sanitary Sewer Overflow Response (SOG) NUMBER E001 DATE EFFECTIVE: 3/1/2013 VERSION: 1 DATE LAST REVISED: 1/1/2013 REVEIWED: Annually PURPOSE This Sanitary Sewer Overflow Response protocol communicates the process to be followed in cases of wastewater system failure or accidental release that could impact health and/or the environment. It will determine the processes to take to ensure continuity of operations. AUTHORITY The Environmental Health & Safety Department has authority to make decisions to protect the health, safety and environmental quality of the campus community and surrounding area. •. Chain of Command - The Incident Commander handles all operational decisions - The Public Information Officer (PIO) will communicate with the media CHECKLIST Emergency Checklist for Sanitary Sewer Overflow • A call to report a sanitary sewer overflow may come from three sources: 1. University Police 2. The Physical Plant 3. A citizen response/ occupant response • Priorities for spill response are 1. Containment. 2. Confinement 3. Recovery 4. Disinfection If the call is received from a building occupant, note the date and time of call and ask the person: 1. Location (building, room number, parking lot, etc.). 2. Description of the problem, including magnitude of the spill 3. The status of the Location (building occupied, parking lot empty, etc) 4. Where is it coming from (manhole, drain, toilet, sink, etc.) 5. How long they have observed the overflow 6. what other persons are on scene or have been called 7. Response: • Take rubber boots, gloves, Tyvek suit (or similar), digital camera and other PPE as required • If :the spill is threatening a creek or storm drain, take water -absorbing booms or other large scale spill response supplies • Notify the EH&S Manager on Call of the incident and response activities underway; coordinate the need for notification of building management with the EH&S Manager • Upon arriving at the site, identify yourself with the Incident Commander and/or emergency personnel on site • Ensure Physical Plant on -call plumber and Physical Plant administration have been contacted. Ensure perimeter is secured to prevent contact with the water • Determine the Operator in Responsible Charge (ORC) via Physical Plant • Estimate flow rate if possible • Determine if flow is possible to storm sewer and therefore surface water bodies • Determine if flow is damaging building components or impacting use of the facility' • Once situational awareness is gained, consider senior management notification, if possible, through EH&S manager -on -call • If the spill affects interior building space, determine the effect on occupants and whether alternate locations will be necessary until restoration is complete.
