Dear Mr. Caple:
Reference is made to your request for rescission of the non -discharge groundwater remediation permit
referenced above. Based upon information provided to staff of the Central Office and the Winston-Salem
Regional Office, your request to rescind this permit is approved. Therefore, in accordance with your request,
Permit WQ0000965 is rescinded with an effective date of this letter.
NOTE: Any monitoring wells remaining on -site that are going to be continued to be used for groundwater
quality sampling will need to be abandoned per 15A NCAC 02C .0240 when this project is complete and
no longer needed.
If in the future, you wish to operate a groundwater remediation system, you must first apply for and receive
a new permit. Operating an injection system without a valid permit may result in the assessment of civil
penalties and/or the use of other enforcement mechanisms available to the state. If it would be helpful to
