Enclosed is a report for the inspection performed on February 19, 2004 by Ms.
Tonja Springer. Where deficiencies are cited in this report, a response is required as well
as for all lettered comments and/or recommendations. Within thirty days of receipt, please
supply this office with a written item for item description of how these deficiencies,
comments and/or recommendations were corrected. If the deficiencies cited in the
enclosed report are not corrected, enforcement actions will be recommended. For
certification maintenance, your laboratory must continue to carry out the requirements set
forth in 15A NCAC 2H .0800.
Copies of the checklists completed during the inspection may be requested from this

I. INTRODUCTION:
This laboratory was inspected to verify its compliance with the requirements of 15A NCAC 2H .0800 for
the analysis of environmental samples..
II. GENERAL COMMENTS:
The inspection consisted of reviewing data for NPDES # NC0049778
Guidance documents were given during the inspection for all certified parameters.
Comment: An amendment form was submitted for a change of address and to delete Temperature.
A. Comment: For all the certified parameters, the laboratory is documenting the collection time and
analysis time on the C.O.C. and not on the benchsheet.
Requirement: Certified Data must consist of date collected, time collected, sample site, sample
collector, and sample analysis time. The field benchsheets must provide a space for the signature or
initials of the analyst, and proper units of measure for all analyses. Reference: 15A NCAC 2H .0805

III. DEFICIENCIES, REQUIREMENTS, COMMENTS AND RECOMMENDATIONS,:
Total Residual Chlorine:
A. Comment: The laboratory is analyzing a blank, 0.25 .mg/I and 0.50 mg/L each analysis day.
However, the daily check standards do not meet the requirement of 3 standards daily or an annual
curve. The blank is not considered a standard.
Requirement: The lab may demonstrate the linearity of the instrument by analyzing a three point
documentation daily, or they may analyze five standards once per year and analyze a single standard
once per day after the annual verification. Please submit a copy of the curve with the response.
Requirement: For analytical procedures requiring analysis of a series of standards, the concentrations
of these standards must bracket .the concentration of the samples analyzed. One of the standards
must have a concentration equal to the laboratory's lower reporting concentration for the parameter
involved. Reference: 15A NCAC 02H .0805 (a) (7) (I).

Comment: Purchased "Gel -type standards may be used for daily calibration verification only. The
standard will need to be verified annually with the standard curve. The obtained reading will be used
as the true value. It is recommended that the check standard be at mid -range or higher concentration
and should be within 10% of the known value.
B. Comment: The laboratory is reporting the actual obtained numbers on the DMR.
Comment: The laboratory will need to report <250 pg/L on the DMR, until a curve is established. The
results reported on the DMR for the previous weeks for the month of February, will need to be changed
to reflect the lower reporting limit of <250 pg/L. Once a curve is established, a lower reporting limit of
<0.1 mg/L or <100 ug/L can be reported.
Settleable Solids:
D. Comment: The laboratory documents the results directly on the COC,

E. Recommendation: It is recommended that the start and finish time of analysis be documented on
the benchsheet to show that the time required by the method is met.
Requirement: Settle for 45 min, gently agitate sample near the sides of the cone with a rod or by
spinning, settle 15 min longer and record volume of settleable solids in the cone as milliliters per liter.
Reference: 2540 F (3) (a)18th Edition Standard Methods.
IV. PAPER TRAIL INVESTIGATION:
A paper trail was conducted at this facility. This consisted of comparing laboratory bench data with
DMR reports. There were no transcription errors detected.
V. CONCLUSIONS:
Correcting the above cited comments and implementing the recommendations will help this lab to
produce quality data and meet certification requirements. Please respond to all lettered comments
and recommendation.

We have scheduled an on -site inspection of your field laboratories for approximately 11:00 a.m., January
28, 2004. The scope of the inspection will include all methods marked on the application for NC
Wastewater/Groundwater Field Laboratory Certification. The purpose of the inspection will be to verify the
laboratory's documented quality control/quality assurance procedures and adequacy in meeting the requirements
of 15A NCAC 2H .0800.
Please have available the DMR report sheets and corresponding bench data available for the months of
July, September, and December of the year 2003 for us to review. Please have available all of the field logs,
corresponding logbooks, equipment using for the analysis and standards for all facilities that have reportable
data. If further data or report sheets are required, we will notify you during the inspection. These data sets will be
used to assess the quality of your data during the inspection.
