Box 218 Monroe, North Carolina 28110 SUBJECT: Union County Crooked Creek Permit No. NC0069841 Authorization to Construct Wastewater Treatment Facilities EPA Project No. C370379-08 Union County Dear Mr. Munn: The final plans and specifications for the subject project have been reviewed and found to be satisfactory. Authorization is hereby granted for the construction of a new 0.65 MGD wastewater treatment facility to consist of an influent pump station with three (3) 600 GPM submersible pumps, soil type odor control equipment and controls; approximately 1,300 lineal feet of 10-inch force main, manual bar screen, dual oxidation ditches each with a volume of 487,500 gallons and dual 40 horsepower rotors; dual 38 foot diameter secondary clarifiers, tertiary sand filters with dual cells having a total filter surface area of 480 square feet, chlorination equipment, a 13,500 gallon chlorine contact basin, chemical (S02) dechlorination equipment, an effluent flow meter, a cascade postaeration device, a 200,000 gallon aerobic sludge digester with a 30 horsepower floating aerator, a sludge transfer station with dual 200 GPM progressive cavity pumps and polymer feed equipment, 22,320 square feet of conventional sludge drying beds, a septage dump station with a 2,000 gallon septage holding tank, a lime feed station, associated yard pipe, site work and electrical work. Do not proceed with construction until the Authorization -to -Award package, EEO, and MBE documentation/certification have been reviewed, and you are in receipt of our approval, if Federal funding is desired for project construction. The permittee shall employ an actively State certified wastewater treatment plant operator to be in responsible charge of the wastewater treatment facilities. Such operator must hold a valid State Certificate of the grade at least equivalent to the classification assigned to the P.O. Box 27687, Raleigh, North Carolina 27611-7687 Telephone 919-733-7015 - 2 - wastewater treatment facilities by the Certification Commission. When a new chief operator must be obtained to be in responsible charge of the new facilities, it is required that this be done by the time the construction of the facilities is 50% complete. The Authorization -to -Construct is issued in accordance with Part =II, Paragraph C of NPDES Permit No. NC0069841, issued July 16, 1987 and shall be subject to revocation unless the wastewater treatment facilities are constructed in accordance with the approved plans, specifications, and supporting documents; and are in compliance with the conditions and limitations specified in Permit No. NC0069841. The permittee shall have an.acceptable sludge analysis and a sludge E.P. toxicity analysis which have been approved by the Division of Health Services, Solid Waste Management Branch prior to the disposal of any sludge from the new wastewater treatment plant. Copies of the analyses, supporting documents, and approvals shall be submitted to the Division of Environmental Management's Water Quality Section and the Mooresville Regional Office. You are reminded that it is mandatory for the project to be constructed in accordance with the North Carolina Sedimentation Pollution Control Act, and, when applicable, the North Carolina Dam Safety Act. In addition, the specifications must clearly state what the contractor's responsibilities shall be in complying with these Acts. The disposing, selling or free distribution of wet, dried, composted sludge or sludge -like materials from wastewater facilities without prior review and approval by this Division is illegal. One (1) set of final approved plans and specifications is to be forwarded to you, one (1) set to the Environmental Protection Agency (EPA), and one (1) set to the Corps of Engineers.
