10/17/14:
Received pc from consultant wants to maintain industrial based permit, primary contaminated sw and gw, has one
current industrial source. Average flow is less than 0.4 MGD, wants to reduce daily monitoring, current permitted flow =
3.9 MGD. Discussed monitoring requirements at different grades. Grade II looks like 0.5 MGD would be applicable and
justifiable, suggested applying for this in renewal. Discussed second tier for future growth, not sure we can but can
certainly ask for this. In past cases inability to characterize the waste resulted in not granting extra tier. Need to provide
data associated with waste to assist with permit renewal.

Wren delivered renewal app to my desk.
1/22/15:
Sonya requested and I provide all app docs so she could add to NPDES File. Told Sonya do not know if BIMS was
updated or receipt ack let sent.
2/20/15:
Wes called to discuss issues with this facility, Wes to include additional recommendation in inspection report he is now
preparing. Permittee is wanting confirmation we had received app, checked file we had original ack let dated Jan
26,2015. Wes provide permittee email address and I sent email to permittee with copy of ack letter attached.
7/9/15:
Permittee called, wanted update on renewal status. Explained permit on backlog and is part of a current review of the
shared nutrients condition that is being developed for the facility using the same nutrients (Belmont, Charlotte Utility).
Unable to give tentative date. Reviewed recent history of pipe repairs.
