This permit is to inject Adventus EHC and ZVI to remediate groundwater contaminated with chlorinated volatile organic compounds at the One Hour Cleaners, 1514 S. Main St., Laurinburg, Scotland County, NC 28352. This permit shall be effective from the date of issuance until February 28, 2013, and shall be subject to the conditions and limitations stated therein, including the requirement to submit a final project evaluation as stated in PART VII — MONITORING AND REPORTING REQUIREMENTS. Please read the entire permit to ensure that you are aware of all compliance requirements of the permit. Please note that the monitoring plan as originally presented in the application has been modified per the Additional Information Letter received March 11, 2011, by the Division of Water Quality. You will need to notify this office and the Fayetteville Regional Office by telephone 48 hours prior to initiation of operation of the facility. In order to continue uninterrupted legal use of the injection facility for the stated purpose, you must submit an application to renew the permit 120 days prior to its expiration date. In response to your request, Hart & Hickman, PC proposes installation of one observation well in the location shown in the revised Figure 2B. The well will be constructed as described in the revised Monitoring Plan and will be installed prior to conducting the pilot -scale injection. Based on the groundwater plume map, this well is located downgradient of the injection area in the prominent direction of contaminant migration. Enclosed please find a revised copy of the Monitoring Plan (Appendix F) and Figure 2B (Appendix H) for insertion into the previously submitted UIC Application. In regards to the proposed monitoring plan for this injection project, there is in essence only one monitoring well (MW-6) downgradient of the proposed injection. MW-4 is in the injection area and MW-3 is cross - gradient to slightly upgradient. Since MW-6 is approximately 60 feet north it will notbe useful to assess groundwater remediation effectiveness or potential plume displacement because groundwater flow rate is approximately 10 feet per year. With a flow rate of 10 feet per year, any change in groundwater quality originating from injection would not appear in the well for 6 years (providing the VOCs flow at the same rate as groundwater). Therefore, in view of the fact groundwater velocity is only approximately 10 feet per year, we would like to request an additional monitoring well be installed downgradient outside the injection area/ROI and at a distance sufficient to evaluate the injection effectiveness; say, approximately 5 to 15 feet. Please submit 2 copies of your responses with any corrected text pages/figures (same scale and page size) for insertion into the previously submitted UIC Application within 30 calendar days of receipt of this letter. Send one copy to my attention at the Central Office and the other copy to the attention of Jun Barber at the Fayetteville Regional Office.
