Please find the subject groundwater monitoring report attached. This includes the sampling and reporting
requirements of the above referenced permit, Part V, Section B 1 as well as the reporting requirements of
the overall Tank Farm Remedial Action Plan. Enclosed are: a) Form GW-59 for injection well CIW7 and
monitor well MWTF 1 CR, b) piezometer level monitoring field sheet, and c) the summary operational and
maintenance report. Also included is a chronology of events associated with the July sampling event and
our efforts to troubleshoot the system.

PCS PHOSPHATE - TANK FARM REMEDIAL ACTION PLAN
QUARTERLY OPERATIONAL AND MAINTENANCE REPORT FOR
THE INJECTION AND WITHDRAWAL WATER WELLS
May 2005 — July 2005
The Tank Farm Remediation System consists of a series of seven- (7) injection water wells located on the
south side of the Tank Farm Area and a series of fourteen (14) withdrawal water wells located on the north
side of the Tank Farm Area.
Injection Water Well System
The injection water well system operated with the DPW Water - Ultraviolet Light Disinfection system
during the period.
Daily operational data are collected by the Shipping Department and reported to the Environmental Affairs
Department. The total monthly and quarterly volumes of water injected as well as the month did water
level in the Water Level Control Tank are:

Withdrawal Well System
The withdrawal water well pumps are monitored daily by the area Maintenance Department and
preventative maintenance work is performed as needed. The total monthly and quarterly withdrawal water
volumes for the period are:

Some additional short periods of planned downtime for preventative maintenance, pipe header cleaning,
and troubleshooting on individual pumps in the fourteen well system were experienced. Details of these
activities are:
Confined Sand Formation Withdrawal Well (SWW) Pumps
On 5/2 flushed all wells. On 5/5 flushed all ground lines. On 5/11 flushed all pumps. On 5/24 flushed all
wells. On 6/1 flushed all wells. On 6/7 flushed all wells. On 6/8 flushed all ground lines. On 6/14 flushed
all ground lines. On 6/21 serviced air compressor. On 6/22 flushed all pumps. On 6/28 flushed all pumps.
On 7/7 flushed all pumps. On 7/13 flushed all pumps. On 7/19 flushed all pumps. On 7/22 flushed all
ground lines. On 7/29 flushed all pumps.
SWW-1: On 5/17 replaced pump.
SWW-2:

SWW-4: On 5/3 acid washed well. On 5/6 changed counter. On 6/22 shook pump.
SWW-5: On 5/31 reset counter. On 6/13 replaced pump.

Croatan Formation Withdrawal Well (CWW) Pumps
CWW-1: On 7/6 changed counter.
CWW-2:
CWW-3: On 7/1 repaired air -line. On 5/16 reset counter. On 5/17 pulled pump and reset. On 5/18 air
blasted well and replaced section of air line.
CWW-4:
CWW-5: On 6/27 repaired pump. On 7/7 replaced pump.
CWW-6:
CWW-7: On 5/9 replaced counter.

Tank Farm Remediation System
7/21/2005
• The UV system was disassembled and inspected.
• There were no apparent physical problems with the UV lamps or with the
glass coverings.
• There was no apparent coating, fouling, scale or film on the glass
covering or the bulbs.
• The coverings were wiped clean with a white towel, no indication of
anything was found on the glass.
• The bulbs were changed and the UV portion of the system was
reassembled.
7/25/2005
• Sterilization of the Tank Farm Remediation System filtration units and
UV treatment units was completed.
7/26/2005
• Sterilization of the Tank Farm Remediation System filtration units and
UV treatment units was repeated.
7/29/2005
• The MWTF-CR, CIW 7 and WLCT were sampled for total coliform.
The sample results were respectively 2 / 100 ml, > 600 / 100 ml and 140 /
100 ml.
8/3/2005 through 8/5/2005
• Sterilization of the Tank Farm Remediation System filtration units and
UV treatment units was repeated.
8/8/2005
• A sample was taken following UV Treatment with both UV units
operating with the flow controlled at 14 GPM. The total coliform was 1 /
100ml.
Tank Farm Remediation System
• A sample was taken following UV Treatment with only the A UV unit
operating with the flow controlled at 14 GPM. The total coliform was 9 /
100 ml.
• A sample was taken following UV Treatment with only the B UV unit
operating with the flow controlled at 14 GPM. The total coliform was <
1/100ml.
• A sample was taken following UV Treatment with both UV units
operating with the flow controlled at 19 - 20 GPM. The total coliform
was 3/100ml.
8/10/2005
• A sample was taken following UV Treatment with both UV units
operating with the flow controlled at 14 GPM. The total coliform was 49
/l00ml.
• A sample was taken following UV Treatment with only the A UV unit
operating with the flow controlled at 14 GPM. The total coliform was 7 /
l00 m1.
• A sample was taken following UV Treatment with only the B UV unit
operating with the flow controlled at 14 GPM. The total coliform was
31 / 100 ml.
• A sample was taken following UV Treatment with both UV units
operating with the flow controlled at 19 - 20 GPM. The total coliform
was 37/l00ml.
A meeting is scheduled for September 1, 2005 with the vendor that supplied
the filtration and UV equipment. The following action steps will be
discussed and evaluated.
1. Replacing the filter media.
2. Installation of additional and / or larger UV lamps.
3. Operation of the UV lamps in series.
4. Installation of a backwash system that will utilize sterilized water.
