monitor well MWTF 1 CR, b) piezometer level monitoring field sheet, and c) the summary operational and
maintenance report.

QUARTERLY OPERATIONAL AND MAINTENANCE REPORT FOR
THE INJECTION AND WITHDRAWAL WATER WELLS
November 2005 — January 2006
The Tank Farm Remediation System consists of a series of seven- (7) injection water wells located on the
south side of the Tank Farm Area and a series of fourteen (14) withdrawal water wells located on the north
side of the Tank Farm Area.
Injection Water Well System
The injection water well system operated with the DPW Water - Ultraviolet Light Disinfection system
during the period.
Daily operational data are collected by the Shipping Department and reported to the Environmental Affairs
Department. The total monthly and quarterly volumes of water injected as well as the month end water
level in the Water Level Control Tank are:

Withdrawal Well System
The withdrawal water well pumps are monitored daily by the area Maintenance Department and
preventative maintenance work is performed as needed. The total monthly and quarterly withdrawal water
volumes for the period are:

Some additional short periods of planned downtime for preventative maintenance, pipe header cleaning,
and troubleshooting on individual pumps in the fourteen well system were experienced. Details of these
activities are:
Confined Sand Formation Withdrawal Well (SWW) Pumps
SWW-1: On 11/2, 11/8, 11/16 and 11/30 flushed out pumps. On 11/16 and 11/30 flushed lines. On 12/5,
12/15, 12/20 and 12/27 flushed out pumps and lines. On 1/11, 1/17, 1/24 and 1/30 flushed out pumps and
lines. On 12/19 Replaced pumps.
SWW-2: On 11/2, 11/8, 11/16 and 11/30 flushed out pumps. On 11/16 and 11/30 flushed lines. On 12/5,
12/15, 12/20 and 12/27 flushed out pumps and lines. On 1/11, 1/17, 1/24 and 1/30 flushed out pumps and
lines. On 1/3 replaced counter. On 1/13 replaced counter. On 1/18 adjusted counter.
SWW-3: On 11/2, 11/8, 11/16 and 11/30 flushed out pumps. On 11/16 and 11/30 flush
12/15, 12/20 and 12/27 flushed out pumps and lines. On 1/11, 1/17, 1/24 and 1/30 flush
lines.
