monitor well MWTF1 CR, b) piezometer level monitoring field sheet, and c) the summary operational and
maintenance report.

QUARTERLY OPERATIONAL AND MAINTENANCE REPORT FOR
THE INJECTION AND WITHDRAWAL WATER WELLS

The Tank Farm Remediation System consists of a series of seven- (7) injection water wells located on the
south side of the Tank Farm Area and a series of fourteen (14) withdrawal water wells located on the north
side of the Tank Farm Area.
Injection Water Well System
The injection water well system operated with the DPW Water - Ultraviolet Light Disinfection system
during the period.
Daily operational data are collected by the SPA/Fertilizer Department and reported to the Environmental
Affairs Department. The total monthly and quarterly volumes of water injected as well as the month end
water level in the Water Level Control Tank are:

Withdrawal Well System
The withdrawal water well pumps are monitored daily by the area Maintenance Department and
preventative maintenance work is performed as needed. The total monthly and quarterly withdrawal water
volumes for the period are:

Some additional short periods of planned downtime for preventative maintenance, pipe header cleaning,
and troubleshooting on individual pumps in the fourteen well system were experienced. Details of these
activities are:
Confined Sand Formation Withdrawal Well (SWW) Pumps
On 6/7, 6/16, 6/21, 7/12, 7/18 and 7/25 flushed out pumps and lines on each of the Confined Sand
Formation Withdrawal Well (SSW) and the Croatan Formation Withdrawal Well (CWW) Pumps. On 5/26
changed oil and filters in the system air compressor.
SWW-1: On 7/6 reset pump.
SWW-2: On 6/7 reset pump.
SWW-3: On 6/6 changed counter.
SWW-4:
SWW-5: On 5/22 flushed pump.
SWW-6: On 5/22 changed pump. On 6/6 changed counter.
