Please find the subject groundwater monitoring report attached. This includes the sampling and reporting
requirements of the above referenced permit, Part V, Section B 1 as well as the reporting requirements of
the overall Tank Farm Remedial Action Plan. Enclosed are: a) Form GW-59 for injection well CIW7,
Form GW-59 for the WLCT and Form GW-59 for monitor well MWTFICR, b) piezometer level
monitoring field sheet, and c) the summary operational and maintenance report.
Note: Inadequate head was available in CIW7 during the sampling event. The injection well water was
sampled upstream from the WLCT. Historical sampling shows that data collected from CIW7 and WLCT
are identical.

QUARTERLY OPERATIONAL AND MAINTENANCE REPORT FOR
THE INJECTION AND WITHDRAWAL WATER WELLS
November 2010 — January 2011
The Tank Farm Remediation System consists of a series of seven- (7) injection water wells located on the
south side of the Tank Farm Area and a series of fourteen (14) withdrawal water wells located on the north
side of the Tank Farm Area.
Injection Water Well System
The injection water well system operated with the DPW Water - Ultraviolet Light Disinfection system
during the period. New UV tubes and bulbs were installed on December 2, 2010.
Daily operational data are collected by the Shipping Department and reported to the Environmental Affairs
Department. The total monthly and quarterly volumes of water injected as well as the month end water
level in the Water Level Control Tank are:

Withdrawal Well System
The withdrawal water well pumps are monitored daily by the area Maintenance Department and
preventative maintenance work is performed as needed. The total monthly and quarterly withdrawal water
volumes for the period are:

Some additional short periods of planned downtime for preventative maintenance, pipe header cleaning,
and troubleshooting on individual pumps in the fourteen well system were experienced. Details of these
activities are:
Confined Sand Formation Withdrawal Well (SWW) Pumps
On 11/3 unplugged SWW Header. On 12/8 changed oil filter on air compressor. On 1/26 the main water
meter was repaired.

new counter.
