NOTE: GW-59 Rev 03/2000 Ros Si. ature ith - Man er, Environmental Affairs me and Title - Please type r pri ermitt e (or Authorized Agent) PCS PHOSPHATE - TANK FARM REMEDIAL ACTION PLAN QUARTERLY OPERATIONAL AND MAINTENANCE REPORT FOR THE INJECTION AND WITHDRAWAL WATER WELLS May 2004 — July 2004 The Tank Farm Remediation System consists of a series of seven- (7) injection water wells located on the south side of the Tank Farm Area and a series of fourteen (14) withdrawal water wells located on the north side of the Tank Farm Area. Injection Water Well System The injection water well system operated with the DPW Water - Ultraviolet Light Disinfection system throughout the period. Daily operational data are collected by the Shipping Department and reported to the Environmental Affairs Department.
