Dear Mr. Sussler:
Agra Environmental, Inc. (Agra) is currently conducting injection activities at the above
listed site. As stated in the Permit Application, a slurry mixture of water and Klozur CR
(75 gallons of approximately 27.60 % w/w Klozur CR per injection point) were injected
into each injection well. However, the injection wells were not able to accept the total
volurne of chemical as proposed in the application.
As such, Agra proposes to excavate three (3) trenches, down to below the local water
table, directly downgradient of the injection wells (as show in the attached Figure). The
remaining volume of chemical mixture will then be injected directly into these trenches.
Once the chemical has been fully dispersed into the groundwater, the trenches will be
filled back in.
It is our understanding that this procedure would not require an additional permit to be
obtained. Please let us know if this modification to the existing permit is acceptable.
Agra has scheduled to conduct these activities early next week; as such a fast response

Dear Mr. Crumpler:
In accordance with the application received on July 13, 2010, we are forwarding permit number W10600062. This permit
is to inject Klozure CR to remediate groundwater contaminated with petroleum products at the above referenced address.
This permit shall be effective from the date of issuance until September30, 2012, and shall be subject to the conditions
and limitations stated therein, including the requirement to submit a final project evaluation as stated in PART VII —
MONITORING AND REPORTING REQUIREMENTS. Please read the entire permit to ensure that you are aware of all
compliance requirements of the permit. Please pay special attention to the following Parts in the Permit:
Part VII.1- The proposed monitoring plan included in the application, shall be followed except that monitoring well MW-
8 shall be added to the monitoring plan and the following field parameters will be obtained: temperature, pH (field),
conductivity, DO, and ORP.
You will need to notify this office and the Fayetteville Regional Office by telephone 48 hours prior to initiation of
operation of the facility. In order to continue uninterrupted legal use of the injection facility for the stated purpose, you
must submit an application to renew the permit 120 days prior to its expiration date. Please contact me at 919-715-6166
