On behalf of Hamilton Beach0Proctor-Silex (HBPS), URS Corporation — North Carolina
(URS-NC) is providing this letter to document the extension of the performance
monitoring period for the ZVI Pilot Test being conducted at the HBPS facility in
Washington, NC (UIC Permit No. WI0700035). Per our discussion on 9 January 2002,
due to the drought experienced in North Carolina during the second half of 2001, the
water table in the upper hydrogeologic unit at the site has declined excessively. URS-NC
has not been able to gather a sufficient number of groundwater samples in the upper
hydrogeologic unit necessary to track the performance of the in situ pilot test. URS-NC
will extend the monitoring period until the groundwater table has recovered and adequate
performance monitoring may be completed. This additional monitoring should be
completed in the Spring of 2002.

Dear Mr. Kuhar:
In accordance with the application submitted on August 20, 1999 we are forwarding Permit No.
WI0700035. This permit is for a Pilot Study to inject a slurry containing zero valent iron (ZVI),
molasses, water and a thixotropic polymer to enhance reductive dehalogenation of the dissolved
chlorinated solvent contamination at 234 Springs Road, in Washington, North Carolina.
This permit shall be effective from the date of issuance until September 30, 2003, and shall be
subject to the conditions and limitations stated therein, including the requirement to submit a final
project evaluation as stated in PART VII - MONITORING AND REPORTING REQUIREMENTS.
You will also need to notify this office by telephone 48 hours prior to initiation of injection at this
facility. In order to continue uninterrupted legal use of this injectant for the stated purpose, you
should submit an application to renew the permit three months prior to its expiration date

Per your request, we will sample and analyze groundwater samples for
acrylamide during the pilot test as follows:
Two additional sampling implants will be installed approximately 25 feet
downgradient of each of the two pilot test injection locations. These
implants will be sampled at 5 weeks and 10 weeks post injection. Analysis
will be done by SW-846 Method 8032A for acrylamide

Dear Mark:
Per your request, I am writing to verify that the ZVI Pilot Test we have proposed to conduct at
the Washington, NC site is very similar to the ZVI Pilot Test we recently conducted at the
Abbott Labs facility in Laurinburg, NC (UIC Permit No. WI0600010). The remediation
contractor, application procedures, and ZVI slurry contents are identical for both of these pilot
test applications. The iron slurry will consist of ZVI, molasses, water, and the same thixotropic
agent to control slurry viscosity.
Attached are figures documenting the locations of the application and monitoring points for the
pilot -scale test at the Washington facility. The pilot test injection taking place in the upper
hydrogeologic unit (Unit A) will inject the iron slurry from approximately three feet to ten feet
below ground surface (bgs). Eight temporary monitoring wells will be installed for the Unit A
pilot test as depicted in Figure 1. These wells will be screened from approximately five to ten
feet bgs. The pilot test injection taking place in the lower hydrogeologic unit (Unit B) will inject
the iron slurry from approximately 15 feet to 35 feet below ground surface (bgs). Nine
temporary monitoring wells will be installed for the Unit B pilot test as depicted in Figure 1.
These wells will be screened at staggered depths within Unit B (between 20 and 35 feet bgs) to
monitor potential variations in contaminant concentrations over depth.

Mario,
Attached below is a replacement page for "Attachment H: Pilot -Scale Test
Monitoring" of the ZVI UIC permit application for the Washington ZVI
injection. The original permit application is now almost two years old,
and we have modified and improved the pilot test monitoring plan in this
time. Upon your approval, I will take this replacement page to Mark Pritzl
of the NCDENR UIC group, and have him replace the previous "Attachment H"
page.
I spoke with Mark Pritzl last week. He said that we could expect to have
the approved permit by the end of August. He also informed me that Meliktu
Fanuel, the head of the UIC group, is leaving in mid -August. Again, Mark
will -be -the -only-UIC—representative-untia-a-replacement-can-be found fox
Meliktu.
I am finalizing the ZVI/Molasses Pilot -Scale Test proposal, and I will send
it to you shortly for your review.

Pilot -Scale Test Monitoring
Newly constructed 1-inch diameter monitoring wells will be utilized to measure
the effectiveness of the treatment during the pilot application. Eight or nine monitoring
wells will be utilized in both hydrogeologic units. These wells will be sampled before,
during, and after the pilot test. Samples will be analyzed for VOCs, ferrous iron, and
chloride. In addition, parameters such as dissolved oxygen, pH, conductivity,
temperature, and oxidation-reduction potential (ORP) will be recorded during the
application process and post -treatment sampling to gauge the progress of the reactions.
The on -site activities associated with the pilot -scale test will be conducted fora period of
approximately 12 weeks.
