Please find the subject groundwater monitoring report attached. This includes the sampling and reporting
requirements of the above referenced permit, Part V, Section B 1 as well as the reporting requirements of
the overall Tank Farm Remedial Action Plan. Enclosed are: a) Form GW-59 for injection well CIW7 and
monitor well MWTF1CR, b) Form GW-59 for each of the fourteen withdrawal wells, c) Form GW-59 for
monitor wells MWTF1A and MWTF1B, d) Form GW-59 for the WLCT, e) piezometer level monitoring
field sheet, and f) the summary operational and maintenance report.

PCS PHOSPHATE - TANK FARM REMEDIAL ACTION PLAN
QUARTERLY OPERATIONAL AND MAINTENANCE REPORT FOR
THE INJECTION AND WITHDRAWAL WATER WELLS
February 2003 — April 2003
The Tank Farm Remediation System consists of a series of seven- (7) injection water wells located on the
south side of the Tank Farm Area and a series of fourteen (14) withdrawal water wells located on the north
side of the Tank Farm Area.
Injection Water Well System
The injection water well system operated with the DPW Water - Anthracite Sand Filter / Ultraviolet Light
(ASF/UV) disinfection system throughout the period.
Daily operational data are collected by the Shipping Department and reported to the Environmental Affairs
Department. The total monthly and quarterly volumes of water injected as well as the month end water
level in the Water Level Control Tank are:

Withdrawal Well System
The withdrawal water well pumps are monitored daily by the area Maintenance Department and
preventative maintenance work is performed as needed. The total monthly and quarterly withdrawal water
volumes for the period are:

Some additional short periods of planned downtime for preventative maintenance, pipe header cleaning,
and troubleshooting on individual pumps in the fourteen well system were experienced. Details of these
activities are:
Confined Sand Formation Withdrawal Well (SWW) Pumps
SWW-1: On 2/5 shook pump to start. On 4/1 shook pump to start. On 4/14 changed pump.

SWW-5: On 4/4 shook pump to start. On 4/14 changed pump.

SWW-7: On 2/10 shook pump. On 2/18 pulled pump and cleaned well. On 3/10 changed pump. On 3/19
acid washed well.
