requirements of the above referenced permit, Part V, Section B 1 as well as the reporting requirements of
the overall Tank Farm Remedial Action Plan. Enclosed are: a) Form GW-59 for injection well CIW7 and
monitor well MWTF1CR, b) Form GW-59 for each of the fourteen withdrawal wells, c) Form GW-59 for
monitor wells MWTF1A and MWTF1B, d) Form GW-59 for the WLCT, e) piezometer level monitoring
field sheet, and f) the summary operational and maintenance report.

QUARTERLY OPERATIONAL AND MAINTENANCE REPORT FOR
THE INJECTION AND WITHDRAWAL WATER WELLS
February 2000 — April 2000
The Tank Farm Remediation System consists of a series of seven (7) injection water wells located on the
south side of the Tank Farm Area and a series of fourteen (14) withdrawal water wells located on the north
side of the Tank Farm Area.
Injection Water Well System
The injection water well system operated with the DPW Water - Anthracite Sand Filter / Ultraviolet Light
(ASF/W) disinfection system throughout the period. On March 9 the system was down from 0915
through 1100. This outage was planned to allow preventative maintenance on the system. During the
outage both units received new bulbs, new ballast, new sensors and each unit was calibrated. The bottom
LED on the #1 Sterilizer was also changed.
Daily operational data are collected by the Shipping Department and reported to the Environmental Affairs
Department. The total monthly and quarterly volumes of water injected as well as the month end water
level in the Water Level Control Tank are:

Withdrawal Well System
The Withdrawal Well System was down for a total of 2.5 hours during the period. On 4-7-00 for 2 hours to
repair a leaking air line and on 4-10-00 for 0.5 hours to change belts. The withdrawal water well pumps are
monitored daily by the area Maintenance Department and preventative maintenance work is performed as
needed. The total monthly and quarterly withdrawal water volumes for the period are:

Some additional short periods of planned downtime for preventative maintenance, pipe header cleaning,
and troubleshooting on individual pumps in the fourteen well system were experienced. Details of these
activities are:
Air Compressor for the Withdrawal Well Pumps
The air compressor was down for 2 hours to repair an air line leak.
Confined Sand Formation Withdrawal Well (SWW) Pumps
SWW-1: On 2/8, 2/25, 3/2 and 4/4 the pump was shaken to start. On 4/7 the pump was replaced. On 4/10
the pump was replaced. On 4/12 and 4/14 the pump was shaken to start. On 4/17 the pump was replaced.
SWW-2: On 3/29 the pump was shaken to start.
SWW-3: Pump operated without incident.
SWW-4: Pump operated without incident.
S W W-5: On 4/28 had to shake pump to start
SWW-6: Pump operated without incident.
SWW-7: Pump operated without incident.
Croatan Formation Withdrawal Well (CWW) Pumps
CW W-1: On 3/29 the pump was replaced. On 4/7 repaired the air line.
CWW-2: On 3/2 the pump was replaced. On 3/29 the totalizer was replaced.
CWW-3: Pump operated without incident.
CWW-4: Pump operated without incident.
CWW-5: On 2/8/ was shaken to start.
CWW-6: On 4/12 the pump was shaken to start.
CWW-7: On 2/15 repaired totalizer. On 3/14 the pump was shaken to start.
