Davis/OE-00-01-00-07 C:\YTRMGMT\REPORTS\1997\OPDATA4.97 PCS PHOSPHATE - TANK FARM REMEDIAL ACTION PLAN QUARTERLY OPERATIONAL AND MAINTENANCE REPORT FOR THE INJECTION AND WITHDRAWAL WATER WELLS FEBRUARY - APRIL 1997 The Tank Farm Remediation System consists of a series of seven injection water wells located on the south side of the Tank Farm Area and a series of fourteen (14) water withdrawal wells located on the north side of the Tank Farm Area. Injection Water Well System The injection water well system operated with the DPW Water - Anthracite Sand Filter/Ultraviolet Light (ASF/UV) disinfection system throughout the period. Only short periods of planned downtime were experienced for preventative maintenance and inlet DPW Water strainer cleaning. Daily operational data are collected by the Shipping Department and reported to the Operations Environmental Department. The withdrawal water well pumps are monitored daily by the area Maintenance Department and preventative maintenance work is performed is needed. The total withdrawal water volumes for the period are: February March Sand Formation 420,198 428,481 Croatan Formation 236,884 262,006 Total Gallons 657,082 690,487 April Ortiv Totals 407,862 257,841 665,703 1,256,541 756,731 2,013,272 Some short periods of planned downtime for preventative maintenance, pipe header cleaning, and troubleshooting on individual pumps in the fourteen well system were experienced. On 3/28 all of the pipe headers were cleaned by flushing with hot water and air. Specific well and pump maintenance activities include: Confined Sand Formation Withdrawal Wells (SWW) SWW-1; On January 27 the pump cycle counter started malfunctioning only counting cycles part of the time and the sensor was replaced on February 1. On February 1 the pump was replaced due to scale build up plugging 90% of bottom strainer. On 2/17 the pump stopped and had to be shaken to start. On 2/18 the pump stopped and was removed for inspection. It was determined that the pump and well had scaled up and required cleaning with HC1. On 2/20 the pump and well were cleaned with HC1 and placed back in service. On 3/10 the pump stopped and was determined to need cleaning. On 3/11 the pump and discharge line were cleaned. On 3/21 the pump was operating slower than normal and it was determined to need cleaning. On 3/31 the pump stopped. On 4/1 the pump was removed and cleaned. On 4/1 the well was cleaned for 26 hours with HC1. On 4/2 the pump and well were placed back in service. On 4/7 the pump stopped and had to be shaken to start. On 4/18 the pump stopped. On 4/21 the pump was repaired and the plugged discharge line to the header was cleaned. SWW-2; On 4/3 the pump stopped. On 4/7 the pump was removed and cleaned. On 4/7 the well was cleaned for 6 hours with HC1 and placed back in service the same day. SWW-3; On 4/29 the pump stopped, was removed and cleaned, and placed back in service. SWW-4; On 2/3 -a loose wire was repaired on the cycle counter. On 3/24 the cycle counter malfunctioned. On 3/26 the counter sensor was cleaned and it began operating properly. On 4/10 the pump stopped. On 4/11 the pump was removed and cleaned. The cam assembly was replaced due to a broken snap ring on the air valve and the pump was placed back in service. SWW-5; On 2/4 the air line to the pump broke off and was repaired. SWW-6; No problems. SWW-7; No problems. Croatan Formation Withdrawal Wells (CWW) CWW-1; The counter sensor was replaced on February 1. On 2/21 the pump stopped and had to be shaken to start. On 3/31 the pump stopped and had to be shaken to start. CWW-2; On 2/3 the cycle counter was working only part of the time. On 2/4 the pump stopped, was removed, cleaned and placed back in service. On 2/11-12 the cycle counter was working only 3 part of the time. On 2/13 the pump was removed, cleaned, and placed back in service with a new cycle counter sensor. On 2/25 the cycle counter was working only part of the time. On 3/25 the pump stopped and had to be shaken to start. On 3/31 the pump stopped and had to be shaken to start. CWW-3; On 3/31 the pump stopped and had to be shaken to start. On 4/1 the pump stopped and was blowing air. On 4/2 the pump was removed and cleaned. On 4/2 the well was cleaned for 5 hours with HC1 and placed back in service the same day. On 4/25 the pump stopped and had to be shaken to start. CWW-4; On 3/7 the pump stopped and had to be shaken to start. On 3/13 the pump stopped and it was determined to need cleaning. On 3/18 the pump was cleaned and a broken snap ring on the cam assembly was repaired. CWW-5; On 3/25 the pump stopped. On 3/26 the pump was removed, cleaned and the cam assembly was replaced due to broken snap rings on both air valves. CWW-6; No problems. CWW-7; On 3/10 the pump stopped and was determined to need cleaning. On 3/11 the pump was cleaned and the cam assembly was replaced due to a broken snap ring on the air valve. 4
