KANE GROUND WATER SECTION NCDENR 1636 MAIL SERVICE CENTER RALEIGH, NC 27699-1636 CERTIFIED MAIL Subject: Underground Injection Control Program Quarterly Groundwater Monitoring Report November 2003 — January 2004 Injection Well Permit #WI0700012 Tank Farm Remedial Action System Beaufort Co. Dear Evan: Please find the subject groundwater monitoring report attached. This includes the sampling and reporting requirements of the above referenced permit, Part V, Section B 1 as well as the reporting requirements of the overall Tank Farm Remedial Action Plan. Enclosed are: a) Form GW-59 for injection well CIW7 and monitor well MWTF 1 CR, b) piezometer level monitoring field sheet, and c) the summary operational and maintenance report. Initial sampling was conducted on January 21, 2004. At this time the WLCT, CIW-7 and MWTF 1C were sampled for coliform. The sample results were 7 / 100 mis for the WLCT, 17 / 100 mis for THE CIW-7 and 290 / 100 mis for the MWTF 1 C. Resampling was conducted on February 4, 2004. At this time the WLCT, CIW-7 and MWTF 1C were sampled for coliform. The sample results were 3 / 100 mis for the WLCT, > 600 / 100 mis for the CIW-7 and < 1 / 100 mis for the MWTF 1 C. The Washington Regional Office was notified of these results and informed that we suspected the system UV sterilizer as the probable cause of the high coliform readings. To test this theory we resampled before and after the UV sterilizer on February 9, 2004. The sample results were received on February 18, 2004 and are as follows, 80 / 100 mis before UV and < 1 / 100 mis after UV. Based on these results it was decided to inject sodium hypochlorite into the system to sterilize the header on February 19, 2004 and resample on February 25, 2004. The connections were made to inject sodium hypochlorite into the system as planned; however conductivity protection would not allow the injection of the sodium hypochlorite into the system at this point. The system will be super chlorinated on February 24, 2004 and the sodium hypochlorite allowed to remain in the system as much as possible for 12 to 18 hours. Following this action the system will be sampled again. Once the sample results have been received they will be forwarded to your office as well as the Washington Regional Office. Injection Water Well System The injection water well system operated with the DPW Water - Ultraviolet Light Disinfection system throughout the period. Daily operational data are collected by the Shipping Department and reported to the Environmental Affairs Department.
