Greetings Gentlemen -
We have done a preliminary review of the above referenced report dated May 2016 and have a few
comments/requests:
• The last sampling event for metals in 2011 shows elevated phosphorous concentrations in monitoring wells 32-
MW-17 and 32-MW-18 when compared to pre -injection
levels,
and no post -injection sampling results for 32-MW-1 due to the well being dry. The elevated
phosphorous is a result from the injection activity in September 2010.
• The last monitoring event for VOCs in June 2013 show exceedances of some parameters including VC in 32-MW-
1 and 32-MW-18, probable daughter products of the remediation resulting from the injection in 2010.
• Per 1SA NCAC 02L .0202, injectants and related reaction byproducts must attenuate to pre -injection
concentrations or groundwater standards, whichever is greater, in order for this permit to be closed. Otherwise,
corrective action may be required.
Therefore, we would like to request a final round of sampling for all parameters to assure the groundwater quality has
met the rule requirements. If the resulting sampling shows GW quality meeting the standards, we can then close out
this permit in our system and send you a Permit Completion Letter. When you send the laboratory results please
provide status of the injection wells, i.e., will the wells be used for future monitoring/injection, plugged and abandoned,
etc. (provide GW-30s if abandoned).

Mr. Prosser:
A recent review of our files indicated several deficiencies for groundwater remediation permit WI0600047 (NC DOT
Priority Site 32). You were listed as the project contact.
Please address the following issues in regards to the subject permit:
1) Records: We need the actual well construction/abandonment records for the injection wells. None were
submitted.
2) Records: According to the permit, an Injection Event Record was to be submitted within 30 days of completing
each injection. No record was submitted.
3) Monitoring Plan: It was unclear as to what wells were to be sampled and what parameters were to be analyzed.
4) Monitoring Plan: Five groundwater sampling events were proposed; one event at one day prior to injection and
then 4 additional events at 1, 3, 6, and 12 months after injection. No groundwater results for any of the events
were submitted.
5) Final Project Evaluation: According to the permit, a Final Project Evaluation was to be submitted within 9
months after completing all injection activities. No such report was submitted.
6) Permit Status: The permit expired on November 30, 2011. The permit needs to be either renewed or rescinded.

Dear Mr. Niver:
In accordance with the application received on August 13, 2009, and subsequent information received December 17,
2009, we are forwarding permit number WI0600047. This permit is to inject Sodium Lactate, Sodium Bicarbonate,
and Sodium Polymetaphosphate solutions to remediate groundwater contaminated with chlorinated aliphatic
solvents at the facility referenced above. This permit shall be effective from the date of issuance until November 30.
2011, and shall be subject to the conditions and limitations stated therein, including the requirement to submit a final
project evaluation as stated in PART VII = MONITORING AND REPORTING REQUIREMENTS. Please read the
entire permit to ensure that you are aware of all compliance requirements of the permit.
You will need to notify this office by telephone 48 hours prior to initiation of operation of the facility. In order to
continue uninterrupted legal use of the injection facility for the stated purpose, you must submit an application to renew

intervals for 32-MW-17 and 32-MW-18 are 10 to 30 ft bsg and 41 to 61 ft bsg, respectively. Sorry for the
confusion.

As required by the Underground Injection Control (UIC) permit (# WI 0400067),
Pyramid Environmental & Engineering, P.C. (Pyramid) notified the Aquifer Protection
Section-UIC Central Office staff and the Winston-Salem Regional Office Aquifer
Protection Section Staff of an injection event. As stated in the new and revised UIC
permit, an Injection Event Record should be submitted to the Aquifer Protection Section
Central Office within 30 days of completing each injection event. The details of this
injection event are summarized in the following letter report.
On December 6 and 7, 2007, an injection was completed at the Market On Main Street
facility (furniture showroom) located at 233 South Main Street in High Point, North
Carolina. Monitoring well/injection wells MW-1, IW-1, and IW-5 were used as injection
points. On December 6, 2007, a total of approximately 90-gallons of Persulfate was
injected between MW-i, IW-1, and IW-5. On December 7, 2007, a total of
approximately 30-gallons of the Persulfate was injected between MW-1, IW-1, and IW-5.

As stated in the permit, monitoring wells MW-2, MW-4, MW-5, SVE-5, and SVE-6 shall
be sampled 30 days after completion of the injection. Monitoring wells MW-2, MW-4,
and SVE-5 will be sampled for VOCs, DO, ORP, pH, temperature, and sulfate.
Monitoring wells MW-2, MW-4, MW-5, and SVE-6 will be sampled for RCRA Metals.
