QUARTERLY OPERATIONAL AND MAINTENANCE REPORT FOR
THE INJECTION AND WITHDRAWAL WATER WELLS
February, 1999 — April, 1999
The Tank Farm Remediation System consists of a series of seven injection water wells located on -the south
side of the Tank Farm Area and a series of fourteen (14) water withdrawal wells located on the north side
of the Tank Farm Area.
Injection Water Well System
The injection water well system operated with the DPW Water - Anthracite Sand Filter / Ultraviolet Light
(ASF/UV) disinfection system throughout the period. Short periods of planned downtime were
experienced for preventative maintenance and inlet DPW Water strainer cleaning.
Daily operational data are collected by the Shipping Department and reported to the Environmental Affairs
Department. The total monthly and quarterly volumes of water injected as well as the month end water
level in the Water Level Control Tank are:

Withdrawal Well System
The Withdrawal Well System operated the entire period. The withdrawal water well pumps are monitored
daily by the area Maintenance Department and preventative maintenance work is performed is needed.
The total monthly and quarterly withdrawal water volumes for the period are:

Some additional short periods of planned downtime for preventative maintenance, pipe header cleaning,
and troubleshooting on individual pumps in the fourteen well system were experienced. Details of these
activities are:
Air Compressor for the Withdrawal Well Pumps
Operation of the air compressor system was normal during this period with no downtime. On 2/26 a faulty
hour meter was replaced.
Withdrawal Well Pump Discharge Piping
On 2/16 the discharge piping header systems from the central valve station to the sump was cleaned by
hydroblasting.
Confined Sand Formation Withdrawal Well (SWW) Pumps
S W W-1: On 2/9 and 11 the pump stopped and was shaken to start. On 3/19 and 4/26 the pump was cleaned
with HC1.
SWW-2: On 3/19 the pump was cleaned with HC1. On 3/22 the pump stopped. On 3/23 a faulty cam
assembly was replaced. On 4/30 the pump was cleaned with HCI.
SWW-3: On 2/9 the pump stopped and was shaken to start. On 3/19 the pump was cleaned with HC1. On
3/22 the pump stopped. On 3/24 the pump was replaced. On 3/27 the pump stopped and a broken cam
assembly ring was found. The cam was replaced and the pump was returned to service. On 3/29 the pump
stopped and the float was replaced. On 4/30 the pump was cleaned with HC1.
SWW-4: On 3/19 the pump was cleaned with HCI. On 3/22 the pump stopped. On 3/23 the pump was
inspected and found with the magnet pulled out of the pump head. The magnet was replaced and the pump
was returned to service.
S W W-5: On 2/8 the pump cycles had reduced from normal and the pump was cleaned with HC1. On 3/19
the pump was cleaned again with HCI.
SWW-6: On 3/31 the pump cycles had reduced from normal and the pump was cleaned with HC1.
SWW-7: There were no problems with this pump during the period.

Croatan Formation Withdrawal Well (CWW) Pumps
CWW-1: On 2/1, 3, and 24 the pump stopped and was shaken to start. On 2/25 the pump stopped, was
cleaned with HC1 and a faulty cam was replaced. On 3/19 the pump was cleaned with HC1. On 4/27 the
pump cycles had reduced from normal. The pump was cleaned and the float was replaced. On 4/30 the
pump was cleaned again with HC1.
CWW-2: On 2/1 the pump stopped and was replaced with a rebuilt pump. On 3/19 and 4/30 the pump was
cleaned with HCI.
CWW-3: On 3/19 and 4/30 the pump was cleaned with HC1.
CWW-4: On 2/23 the pump stopped and was replaced with a rebuilt pump. On 3/19 the pump was cleaned
with HC1.
CWW-5: On 2/8 the pump cycles had reduced from normal and the pump was cleaned with HC1. On 2/12
the pump was not working normally and the bottom check seat was replaced. On 3/19 the pump was
cleaned with HC1.
CWW-6: On 3/31 the pump cycles had reduced from normal and the pump was cleaned with HC1.
CWW-7: On 3/27 the pump cycles had reduced from normal and the pump was replaced with a rebuilt
pump.
