QUARTERLY OPERATIONAL AND MAINTENANCE REPORT FOR
THE INJECTION AND WITHDRAWAL WATER WELLS
May 2000 — July 2000
The Tank Farm Remediation System consists of a series of seven (7) injection water wells located on the
south side of the Tank Farm Area and a series of fourteen (14) withdrawal water wells located on the north
side of the Tank Farm Area.
Injection Water Well System
The injection water well system operated with the DPW Water - Anthracite Sand Filter / Ultraviolet Light
(ASF/UV) disinfection system throughout the period.
Daily operational data are collected by the Shipping Department and reported to the Environmental Affairs
Department. The total monthly and quarterly volumes of water injected as well as the month end water
level in the Water Level Control Tank are:

Withdrawal Well System
The Withdrawal Well System was down for a total of 7.7 hours during the period. The system was down
on 5-9-00 for 1 hour to clean air compressor cooler. The system was down on 6-27-00 for 40 minutes for
compressor maintenance. The system was down 6 hours on 7-12-00 for compressor service. The
withdrawal water well pumps are monitored daily by the area Maintenance Department and preventative
maintenance work is performed as needed. The total monthly and quarterly withdrawal water volumes for
the period are:

Some additional short periods of planned downtime for preventative maintenance, pipe header cleaning,
and troubleshooting on individual pumps in the fourteen well system were experienced. Details of these
activities are:
Confined Sand Formation Withdrawal Well (SWW) Pumps
SWW-1: Pump operated without incident.
SWW-2: Pump operated without incident. -
SWW-3: Pump operated without incident.
SWW-4: On 5/31, 6/5, 6/9, 6/12, 6/13, 6/15, 6/19, 6/20, 6/21 the pump was shaken to start. On 6/21 the
pump was changed.
SWW-5: On 5/16 had to shake pump to start. On 5/19 acid washed the pump. On 5/24, 5/30, 6/8 and 6/12
the pump was shaken to start. On 6/12 the pump was replaced. On 6/13 the pump was shaken to start and
then replaced. On 6/19 and 6/20 the pump was shaken to start.
SWW-6: On 6/6 the pump was shaken to start. On 6/23 the pump was replaced. On 7/20 the pump was
shaken to start.

SWW-7: Pump operated without incident.
Croatan Formation Withdrawal Well (CWW) Pumps
CWW-1: On 6/9 the pump was replaced. On 7/19 the pump was shaken to start.
CWW-2: On 6/16 the pump was replaced.
CWW-3: On 6/7 and 6/8 the pump was shaken to start.
CWW-4: Pump operated without incident.
CWW-5: On 6/28 the pump was shaken to start.
CWW-6: On 5/31 the pump was replaced. On 7/25 the pump was shaken to start. On 7/26 the pump was
cleaned.
CWW-7: On 6/27 the pump was shaken to start. On 7/13 the pump was acid cleaned. On 7/19 the pump
was pulled and cleaned.
