QUARTERLY OPERATIONAL AND MAINTENANCE REPORT FOR
THE INJECTION AND WITHDRAWAL WATER S
May, 1999 — July, 1999 -
The Tank Farm Remediation System consists of a series of seven (7) injection water wells located on the
south side of the Tank Farm Area and a series of fourteen (14) withdrawal water wells located on the north
side of the Tank Farm Area.
Injection Water Well System
The injection water well system operated with the DPW Water - Anthracite Sand Filter / Ultraviolet Light
(ASF/UV) disinfection system throughout the period. Short periods of planned downtime were
experienced for preventative maintenance and inlet DPW Water strainer cleaning. The system was
shutdown two additional times for regular maintenance. On 6/30 the system was shutdown for
approximately five hours to replace a conductivity -sensing probe. On 7/19 the #1 UV Sterilizer was
consistently reading lower than normal intensity. The #2 UV bulb of this Sterilizer was replaced and the
intensity returned to 100%.
Daily operational data are collected by the Shipping Department and reported to the Environmental Affairs
Department. The total monthly and quarterly volumes of water injected as well as the month end water
level in the Water Level Control Tank are:

Withdrawal Well System
The Withdrawal Well System operated the entire period. The withdrawal water well pumps are monitored
daily by the area Maintenance Department and preventative maintenance work is performed is needed.
The total monthly and quarterly withdrawal water volumes for the period are:

Some additional short periods of planned downtime for preventative maintenance, pipe header cleaning,
and troubleshooting on individual pumps in the fourteen well system were experienced. Details of these
activities are:
Air Compressor for the Withdrawal Well Pumps
Operation of the air compressor system was normal during most of the period. On 7/1 a valve on the air
dryer malfunctioned causing the compressor to overheat and shutdown. The I&E Maintenance group was
notified to repair the valve. The system was down for approximately 12 hours.
Confined Sand Formation Withdrawal Well (SWW) Pumps
S W W-1: On 6/28 the pump was cleaned with HC1 and returned to service.
SWW-2: On 6/28 the pump was cleaned with HCI and returned to service. On 7/27 the cycle counter
malfunctioned. It was cleaned/repaired and retumed to service.
SWW-3: On 5/20 the pump stopped and was shaken to start. On 6/28 the pump was cleaned with HC1 and
returned to service. On 7/12 the pump stopped and was shaken to start.
SWW-4: On 6/28 the pump stopped due to pluggage. It was replaced with a rebuilt pump. On 7/6 the
cycle counter malfunctioned and was repaired.
SWW-5: No problems.
SWW-6: No problems.
SWW-7: No problems.
Croatan Formation Withdrawal Well (CWW) Pumps
CWW-1: On 6/28 the pump was cleaned with HCI and returned to service.
CWW-2: On 6/28 the pump was cleaned with HCI and returned to service. On 7/20 the pump was replaced
due to a malfunctioning check valve at the bottom of the pump.
CWW-3: On 6/26 the pump stopped and was blowing air due to a malfunctioning cam assembly. It was
cleaned and the pump was placed back in service. On 6/28 the pump was cleaned with HCl and returned to
service.
CWW-4: On 5/28 the pump stopped due to pluggage. It was replaced with a rebuilt pump.
CWW-5: On 7/6 the cycle counter malfunctioned due to an air leak. This was repaired and the pump was
placed back in service.
CWW-6: No problems.
CWW-7: No problems.

Please find enclosed, the third quarter Tank Farm Remedial Action System groundwater monitoring report
for 1999. This includes the sampling and reporting requirements of the above referenced permit, Part V,
Section B 1 as well as the reporting requirements of the overall Tank Farm Remedial Action Plan.
Enclosed are: a) Form GW-59 for injection well CIW7 and monitor well MWTFICR (R — This monitor
well is a replacement well. The original well was damaged beyond repair.), b) Form GW-59 for each of
the fourteen withdrawal wells, b) Form GW-59 for monitor wells MWTF1A and MWTFIB, c) Form GW-
59 for the WLCT, d) piezometer level monitoring field sheets, and e) the summary operational and
maintenance report.
On May 13, 1999 PCS submitted a request for approval to reduced water level monitoring from monthly to
quarterly. On August 4, 1999 Mr. Keith Starner of the Washington Regional Office, Groundwater Section
approved this request in a telephone conversation with Mr. Terry L. Baker. This report contains the
monthly water levels, but future reports will have quarterly water level data to coincide with the frequency
of this report.
