PCS PHOSPHATE - TANK FARM REMEDIAL ACTION PLAN
QUARTERLY OPERATIONAL AND MAINTENANCE REPORT FOR
THE INJECTION AND WITHDRAWAL WATER WELLS
FEBRUARY, 1998 - APRIL 1998
The Tank Farm Remediation System consists of a series of seven
injection water wells located on the south side of the Tank
Farm Area and a series of fourteen (14) water withdrawal wells
located on the north side of the Tank Farm Area.
Injection Water Well System
The injection water well system operated with the DPW Water -
Anthracite Sand Filter / Ultraviolet Light (ASF/UV)
disinfection system throughout the period. Short periods of
planned downtime were experienced for preventative maintenance
and inlet DPW Water strainer cleaning.
Daily operational data are collected by the Shipping Department
and reported to the Operations Environmental Department. The
total monthly and quarterly volumes of water injected as well
as the month end water level in the Water Level Control Tank

Some additional short periods of planned downtime for
preventative maintenance, -pipe header cleaning, and
troubleshooting on individual pumps in the fourteen well system
were experienced. Details of these activities are:
Confined Sand Formation Withdrawal Wells (SWW)
SWW-1; On 2/9/98 it was determined that the pump and well were
scaled up. The system was cleaned with HC1 and placed back in
operation. On 4/1 a leaking airline was found and repaired.
SWW-2; On 2/12-17 mechanical problems were experienced. On
2/17 the internal float & cam assembly were replaced, the
system was cleaned with HC1 and placed back in operation.
SWW-3; On 2/23, 3/2, 4/1 and 4/15 the pump stopped and had to
be shaken to start. On 3/23 the pump stopped and the air
filter regulator was found plugged. This was repaired and the
system was placed back in operation.
SWW-4; On 2/3 the pump stopped and had to be shaken to start.
On 2/18 the pump stopped and it was determined that the pump
cam assembly magnet had deteriorated. On 2/19 the cam was
replaced, the system was cleaned with HC1. On 2/20 it was
place back in operation. On 2/23 the pump totalizer indicator
was replaced with a new type.
SWW-5; On 3/16-17 the pump experienced mechanical problems.
The system was cleaned with HC1 and the cam assembly hook was
found broken. This was repaired and the pump was placed back
in operation on 3/17. On 4/7 the pump stopped and the
discharge line was found plugged. This was cleaned and the
system was placed back in operation.
SWW-6; On 2/23-3/6 the pump experienced mechanical problems.
On 3/3 the pump inlet check valve was found to be leaking and
the cam assembly & float were replaced. The pump was cleaned
with HC1 and placed back in operation but still was not working
normally. On 3/6 the system was cleaned again and the SS float
was replace with an original epoxy type float and the system
began normal operation.
SWW-7; Normal operation with no problems.
3
Croatan Formation Withdrawal Wells (CWW)
CWW-1; On 2/19 the pum; stopped and had to be shaken to start.
On 3/6-11 the pump experienced mechanical problems. On 3/7 it
was cleaned with HC1. On 3/10 the pump vent valve was
adjusted. On 3/11 the pump SS float was replaced with an epoxy
float and the system began operating normally.
CWW-2; Normal operation with no problems.
CWW-3; On 2/23 the pump stopped. and it was determined to be
scaled up. On 2/24-25 the pump float & air check valve were
replaced, the system was cleaned with HC1 and placed back in
operation.
CWW-4; On 3/30-31 the system was determined to be scaled up,
was cleaned with HC1 and placed back in operation.
CWW-5; Normal operation with no problems.
CWW-6i On 2/6-9 problems were encountered with the pump
totalizer indicator and it was replaced on 2/9. On 2/12
mechanical problems were experienced. The pump float was
replaced, the system was cleaned with HC1 and placed back in
operation. On 2/25-3/2 the pump experienced mechanical
problems. The pump float rod connection to the cam assembly
-was reversed—and—the—pump—sta-r-ted—pumping- normally ._ On 3-/20_
the pump stopped and had to be shaken to start. On 4/14-15 the
pump stopped and the air filter regulator was found plugged.
This was repaired and the system placed back in operation.
CWW-7; On 2/12-13 mechanical problems were experienced. On
2/13 the pump cam assembly was replaced, the system was cleaned
with HC1 and placed back in operation. On 2/18 the pump
stopped and it was determined that the air filter regulator was
plugged and holes were found in the pump. These were repaired
and the system was placed back in operation. On 2/23 and 3/7
the totalizer malfunctioned, was adjusted and returned to
operation. On 3/10 the pump stopped and the air filter
regulator was found plugged. This was repaired and the system
