monitor well MWTF1 CR, b) Form GW-59 for each of the fourteen withdrawal wells, c) piezometer level
monitoring field sheet, and d) the summary operational and maintenance report.

QUARTERLY OPERATIONAL AND MAINTENANCE REPORT FOR
THE INJECTION AND WITHDRAWAL WATER WELLS

The Tank Farm Remediation System consists of a series of seven- (7) injection water wells located on the
south side of the Tank Farm Area and a series of fourteen (14) withdrawal water wells located on the north
side of the Tank Farm Area.
Injection Water Well System
The injection water well system operated with the DPW Water - Ultraviolet Light Disinfection system
during the period.
Daily operational data are collected by the Shipping Department and reported to the Environmental Affairs
Department. The total monthly and quarterly volumes of water injected as well as the month end water
- level in the Water Level Control Tank are:

On 8/18 replaced solenoid and switched back to air compressor.
On 10/12 switched to plant air due to sheave problem.
On 10/22 main discharge header plugged.
On 10/27 switched back to air compressor.
