Please find the subject groundwater monitoring report attached. This includes the sampling and reporting
requirements of the above referenced permit, Part V, Section B 1 as well as the reporting requirements of
the overall Tank Farm Remedial Action Plan. Enclosed are: a) Form GW-59 for injection well CIW7 and
monitor well MWTF 1 CR, b) Form GW-59 for each of the fourteen withdrawal wells, c) Form GW-59 for
monitor wells MWTF1A and MWTF1B, d) Form GW-59 for the WLCT, e) piezometer level monitoring
field sheet, and f) the summary operational and maintenance report.
The initial coliform sample taken from CIW-7 tested 47 / 100 ml. This well is supplied water from WLCT
which tested < 1.0 / 100 ml. CIW-7 was resampled and tested 17 / 100 ml. The Water Level Control Tank
(WLCT) and CIW-7 was chlorinated and the well's outer protective casing was modified to allow direct
access to the well casing for cleaning. CIW-7 was resampled and tested 25 / 100 ml. CIW-7 has been
resampled using a different sampling method to eliminate the possibility of sample contamination. Should
this sample contain coliform the entire injection system will be sanitized.

QUARTERLY OPERATIONAL AND MAINTENANCE REPORT FOR
THE INJECTION AND WITHDRAWAL WATER WELLS
November 2001— January 2002
The Tank Farm Itemediation System consists of a series of seven (7) injection water wells located on the
south side of the Tank Farm Area and a series of fourteen (14) withdrawal water wells located on the north
side of the Tank Farm Area.
Injection Water Well System
The injection water well system operated with the DPW Water - Anthracite Sand Filter / Ultraviolet Light
(ASF/UV) disinfection system throughout the period.
Daily operational data are collected by the Shipping Department and reported to the Environmental Affairs
Department. The total monthly and quarterly volumes of water injected as well as the month end water
level in the Water Level Control Tank are:

Withdrawal Well System
The system was down for 4 hours in January for preventive maintenance on the air compressor. The
withdrawal water well pumps are monitored daily by the area Maintenance Department and preventative
maintenance work is performed as needed. The total monthly and quarterly withdrawal water volumes for
the period are:

Some additional short periods of_planned downtime for preventative maintenance,_pipe header cleaning,_
and troubleshooting on individual pumps in the fourteen well system were experienced. The discharge
header was cleaned between 12/17 and 12/19. Details of these activities are:

SWW-1: On 11/5 repaired pump. On 11/13 repaired air line. On 12/4 pulled and cleaned pump.
SWW-2: On 12/17 cleaned discharge of pump. On 1/22 changed pump.
SWW-3: On 1/9 cleaned pump. On 1/18 pulled and cleaned pump.
SWW-4: On 1/22 pulled and cleaned pump.
SWW-5: On 11/1 changed meter. On 11/13 cleaned pump. On 12/4 changed counter.
SWW-6: On 12/19 pulled pump and cleaned discharge.
SWW-7: On 12/18 pulled pump and changed check valve.
